Position Summary
Registered Practical Nurse, Reactivation Care Centre Unit 4 & 5
Responsibilities
- Provide excellent patient- and family-driven care in a supportive and culturally sensitive manner, collaborating with an interdisciplinary team of care providers with emphasis on assessment, education, prevention, maintenance and restoration of patient health.
- Contribute to ensuring health and safety policies are followed to maintain a healthy environment for patients, staff, physicians, volunteers and families.
- Build strong relationships with peers across the organization and connect effectively with physicians, staff, patients and families.
- Support the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation.

What you will do
- Function to the full scope of nursing practice per the College of Nurses of Ontario.
- Communicate effectively (written and verbal) with patients/clients, families and the health care team.
- Think critically, set priorities and organize work per client/patient needs.
- Demonstrate knowledge of transfers and lifts.
- Commit to ongoing learning, professional development, and continuous quality improvement.
- Maintain proficient computer skills (MS Word, internet searches, data entry and patient information systems).
- Demonstrate self-motivation for continuing education in areas of patient assessment, education and geriatrics.
- Exhibit excellent assessment skills with geriatric care knowledge.
- Display strong organizational, problem-solving, prioritization, time-management and teaching abilities.
- Maintain excellent interpersonal skills to foster good public relations and family-centered care.
Qualifications
- Registered Practical Nursing diploma from a recognized Community College.
- Registered and in good standing with the College of Nurses of Ontario.
- Current Basic Cardiac Life Support (BCLS) certification.
- Minimum 2 years of recent Medical/Surgical and/or clinical nursing experience with older adults, dementia and cognitively impaired population preferred.
- Recent health assessment course.
- Bilingualism in French/English an asset.
