Position Summary

Registered Practical Nurse, Reactivation Care Centre Unit 4 & 5

Responsibilities
  • Provide excellent patient- and family-driven care in a supportive and culturally sensitive manner, collaborating with an interdisciplinary team of care providers with emphasis on assessment, education, prevention, maintenance and restoration of patient health.
  • Contribute to ensuring health and safety policies are followed to maintain a healthy environment for patients, staff, physicians, volunteers and families.
  • Build strong relationships with peers across the organization and connect effectively with physicians, staff, patients and families.
  • Support the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation.

  • Exhibit excellent assessment skills with geriatric care knowledge.
  • Display strong organizational, problem-solving, prioritization, time-management and teaching abilities.
  • Maintain excellent interpersonal skills to foster good public relations and family-centered care.
Qualifications
  • Registered Practical Nursing diploma from a recognized Community College.
  • Registered and in good standing with the College of Nurses of Ontario.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Minimum 2 years of recent Medical/Surgical and/or clinical nursing experience with older adults, dementia and cognitively impaired population preferred.
  • Recent health assessment course.
  • Bilingualism in French/English an asset.
