Qualifications

The Village of Wentworth Heights located in Hamilton Ontario has an exciting opportunity for a Permanent PartTime Registered Practical Nurse (RPN) in LTC to join their team.

As a Registered Practical Nurse (RPN) joining our Village you will :

  • Maintain current registration with the College of Nurses of Ontario.
  • Have existing Heart Save Level of CPR.
  • Demonstrate an ability to prioritize tasks effectively properly plan for and allocate the right amount of time and effort to complete tasks. Remains calm and manages stress appropriately.
  • Deliver safe administration of medication and treatments with a focus on accuracy quality and meeting our residents quality of life requests.
  • Follow plans of care (or service plans) in accordance with the standards of the College of Nurses and able to identify and report any changes in the residents wellbeing to the Charge Nurse or Village leadership team.
  • Have an understanding of relevant policies procedures legislation and regulations applicable to the LongTerm Care sector including the Fixing LongTerm. Care Act 2021 and the Occupational Health & Safety Act (OSHA) of Ontario.
  • Support and embrace our residentcentred social culture and be proud to deliver the highest quality of personal care to our residents.
  • Work as a key member of our Village and Neighbourhood multidisciplinary team to understand residents individual needs and advocate for residents to actively participate in decisions about their care support new resident moveins facilitate transfers to hospital or any changes in resident medical needs.
  • Collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
  • Have proficient communication and computer skills in order to update care plans in the current system.
  • Understand the physical mental and emotional demands of the RPN role.

It would be an asset if our new Registered Practical Nurse (RPN) has :

  • MDS credentials.
  • Experience working with people living with Dementia.
  • Knowledge of the palliative assessments scale.
  • Comfortable supporting residents and their families during endoflife care.
  • Knowledge of relevant policies procedures legislation regulations and best practices applicable to the retirement and longterm care sector (e.g. RAI processes and CIHI supplies equipment ordering etc.)
