Job summary

Baycrest Hospital invites an experienced Registered Practical Nurse (RPN) to join Apotex 7, a Cognitive Support Unit where residents live with varying degrees of cognitive and physical changes. The role involves direct patient care, collaboration with the care team, and responsibility for nursing interventions and assessments.

The successful candidate will hold CNORegistration, BCLS Level C, and demonstrate excellent communication and teamwork skills.

Qualifications

• Current registration, in good standing, from the College of Nurses of Ontario (CNO) is required.
• Current Basic Cardiac Life Support (BCLS) Level C - AED is required.
• Experience in Long-Term Care, day programs, or community setting preferred.
• Psychogeriatric experience and dementia care preferred.
• Training in Gentle Persuasive Approach and U-First preferred.
• Excellent interpersonal, communication and problem solving skills.
• Excellent verbal and written communication skills.
• Basic computer skills – including MDS/RAI, Point Click Care, Word and Excel preferred.

Responsibilities

• In collaboration with RN provides clinical knowledge and acts as resource, delivering comprehensive, client/family centred care from admission through discharge.
• Plan, deliver and evaluate nursing interventions and treatments within the RPN scope of practice.
• Provide client-centred care within the interdisciplinary team to meet personal care, activation, recreation and spiritual needs.
• Perform client assessments.
• Collaborate with the RN to develop and revise the individualized care plan.

Job Summary

Apotex 7 is a Cognitive Support Unit. Residents residing on this floor are living with a wide range of cognitive and physical changes to their abilities – from mild to severe cognitive impairment and from ambulating independently, to being wheel-chair bound.

Key Responsibilities

• In collaboration with RN provides sound clinical knowledge and skills, acts as clinical expert and resource and provides comprehensive, quality client/family centred health care services from admission, through transfers and discharge
• Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments which fall within the RPN’s scope of practice
• Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients
• Performs client assessments
• Collaborates with the Registered Nurse to develop and revise the individualized care plan

Qualifications

• Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required
• Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required
• Experience in Long-Term Care, clients in day programs or residents in a Long-Term Care and Community Setting preferred
• Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours including various types of dementia, delirium, mental health issues preferred
• Certificate in Dementia Care preferred
• Training in Gentle Persuasive Approach and U-First preferred
• Excellent interpersonal, communication and problem solving skills
• Excellent verbal and written communication skills
• Basic computer skills – including MDS/RAI, Point Click Care, Microsoft Word and Excel preferred
