Job Summary:

The Baycrest Adult Day Program is focused on offering older adults a wide range of social, cognitive, physical, and spiritual activities. This program is designed for community-dwelling seniors with physical frailty as well as moderate to severe cognitive impairments.

Responsibilities include but are not limited to:

  • Plans for, delivers, and evaluates nursing interventions, including medication administration as well as treatments which fall within the RPN’s scope of practice.
  • Provides client-centred care within an interdisciplinary care team to meet the personal care, activation, recreation, and spiritual needs of clients with moderate to severe cognitive impairment.
  • Provides leadership and guidance to Program Assistants to meet the clients’ needs and to develop individualized care plans.
  • Documents plans of care, ongoing assessments, interventions, and client progress in the electronic health record.
  • Provides supervision to clients during mealtimes and responds as necessary to any client medical emergencies.

Qualifications include but are not limited to:

  • Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required.
  • Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required.
  • Previous geriatric experience and experience working with clients with moderate to severe cognitive impairment is preferred.
  • Excellent interpersonal, communication, and problem-solving skills.
  • Basic computer skills – Meditech, Microsoft Word, and Excel preferred.
