Job summary

Chartwell Retirement Residences is seeking a Care Coordinator (RPN) to supervise and mentor the care team, ensuring the highest standard of care for residents. You will guide the nursing process, support transitions for new residents, and uphold regulatory compliance within the community.

Ideal candidates hold a current RPN license with good standing and have strong communication skills. PCC knowledge is an asset, and leadership experience is valued for ongoing staff development and quality care.

Qualifications

  • Currently licensed as an RPN with good standing with the CNO.
  • Experience with seniors in a retirement or long-term care setting is an asset.
  • Knowledge of PCC is an asset.
  • Strong verbal and written communication; able to respond to emergencies.

Responsibilities

  • Provide nursing care using the nursing process for residents.
  • Supervise care team and mentor staff.
  • Assess residents during transition into Seasons community and conduct 6-month reassessments per RHRA regulations.
  • Establish priorities and develop person-centered service plans.
  • Ensure PCC-based assessments and compliance with policies and regulations.
  • Support monthly care-related audits and medication administration (PCC knowledge a plus).

Care Coordinator – RPN

Line 4 split MC/ISL 1pm-11pm and 3pm-11pm=66 hours

The Opportunity

We’re looking for a Care Coordinator (RPN) to act as a supervisor providing the highest standard of care services. This role will assist in maintaining a safe and secure environment for residents, their families’ and other staff members while mentoring and providing direction and education to our Resident Guest Attendants for care and other team members.


What You’ll Do


  • Effectively utilizing the nursing process to provide high-quality nursing care to residents

  • Supervisory duties including ensuring excellent care services to Seasons’ residents and providing mentoring/education/guidance to care team on an ongoing basis

  • Assessments of residents moving into Seasons community and following up with residents throughout the transition to community (6-month re-assessments of residents per RHRA regulations, as health care changes)

  • Establish priorities based on individual resident needs

  • Providing person centered service plans

  • Ensuring compliance with Resident Assessments/Service Plans (PCC) while following community specific policies, procedures, and all applicable regulations

  • Risk management by adhering to thorough monthly care related audits

  • Medication administration (PCC knowledge an asset)


What You’ll Bring


  • Experience working with seniors in a retirement or long-term care facility environment is considered an asset

  • Current Registered Practical Nurse License and is in good standing with the CNO

  • Point-Click-Care (PCC) knowledge is considered an asset

  • Strong verbal and written communication skills

  • Must be prepared to respond to emergency situations

  • Strong leadership and supervisory skills in relation to both residents and care staff

  • Current First Aid and CPR certification
