Job summary

Providence Care Hospital in Ontario is seeking a Registered Practical Nurse (RPN) to deliver safe, patient-centered care in a non-acute setting within an inter-professional team. The RPN will support patients and families to manage conditions and transitions while meeting physical, emotional, social and spiritual needs.

The role requires an active RPN license in Ontario, CPR certification, and a college certificate (diploma preferred). A medication certificate post-1985 is also required.

Qualifications

• Active, unrestricted RPN license in Ontario.
• Current CPR certification.
• College certificate; diploma preferred.
• Medication certificate dated after 1985 per OC Nurses.

Responsibilities

• Complete nursing history from patient, family and other sources.
• Develop and record a goal-oriented nursing care plan with patient input.
• Perform interventions with outcomes you can manage.
• Follow prescribed medical regime accurately.
• Re-assess the plan based on goals set.
• Recognize unusual patient responses and consult others as needed.
• Revise nursing care plan based on evaluation and professional judgment.

As a member of an inter-professional team and working under full scope of practice, the role of the Registered Practical Nurse (RPN) at Providence Care is to deliver safe, effective, patient centred care to assigned patients in a non-acute setting as outlined by hospital policies and procedures and the Standards of Nursing Practice of the College of Nurses of Ontario. The RPN will assist patients and their caregivers to manager clinical conditions and transitions and enhance quality of life by meeting the physical, emotional, social and spiritual needs of each person.

Skills and Qualifications Required:

• Active, unrrestricted license to practice with the College of Nurses of Ontario
• Current CPR certificate
• College certificate required, diploma preferred
• Medication Certificate dated after 1985 which meets the requirements of the Ontario College of Nurses, if graduated before 1985

Position Responsibilities:

• Completing a nursing history based on information received from the patient, family and other sources.
• Developing and recording a goal oriented nursing care plan focused on immediate needs in collaboration with the patient.
• Performing interventions for which he/she can manage the outcomes or has resources to manage the outcomes.
• Accurately following the prescribed medical regime
• Re-assessing the plan in terms of goals set.
• Recognizes unusual patient responses and consults with others as required.
• Revising nursing care plan based on evaluation using professional judgment
