Job summary

Compass Community Health in Hamilton, ON is seeking a registered nurse specialized in foot care to provide diabetes foot care services across clinics. The role includes screening, education and direct patient care for diverse communities.

You will join a multidisciplinary team, with on-site foot care training, EMR use, and outreach activities. Full-time and part-time permanent opportunities are available, with a focus on compassionate, community-based care.

Qualifications

  • Current registration with the College of Nurses of Ontario.
  • Completed Basic and/or Advanced Programs in foot care.
  • Completion of Diabetic Program in Foot Care preferred.
  • College diploma and three years of foot care experience in a health care setting.
  • Possess excellent assessment, organizational and problem-solving skills.
  • Availability of transportation to outlying clinics
  • Excellent communication and interpersonal skills
  • Enthusiasm to serve a diverse community
  • Ability to work independently
  • Experience with Electronic Medial Report (EMR) preferred.
  • Proficiency in the use of personal computers and software

Responsibilities

  • Provide foot assessments and care for people living with diabetes at low risk of foot complications.
  • Screen clients according to program criteria and triage to appropriate level of service.
  • Provide education around diabetic foot care.
  • Assist in outreach foot care at various geographic locations.
  • Be a resource person to staff, students and community regarding diabetic foot care.
  • Assist in implementing and providing diabetes foot care health promotion activities.
  • Provide clinical instruction to learners, and RPN students and other health professionals.
  • Screen for foot complications and refer to appropriate care providers.
  • Be a member of the multi-disciplinary team delivering family care.

Compass Community Health offers primary health care services and health promotion programs to diverse communities and individuals.

  • Provide basic foot assessments and care for people living with diabetes who are at low risk of foot complications relating to their disease.
  • Screen clients according to program criteria and ensure that clients referred to the program are triaged to the appropriate level of service.
  • Provide education around the care of the diabetic foot for people living with diabetes.
  • Assist in the provision of outreach foot care at various geographic locations.
  • To be a resource person to the staff, students and community regarding diabetic foot care.
  • To assist in the implement and provision of diabetes foot care health promotion activities in the Community, including increasing the awareness of this service.
  • To provide clinical instruction to learners, and RPN students and other health professionals.
  • Screening for foot complications related to diabetes and referral to appropriate care providers as necessary.
  • To be a member of the multi-disciplinary team of health professionals providing family care to clients and to the community.

Current registration with the College of Nurses of Ontario.

Completed Basic and/or Advanced Programs in foot care.

Completion of Diabetic Program in Foot Care preferred.

College diploma and three years of foot care experience in a health care setting.
