Position: Registered Practical Nurse

Posting information: This posting is for a current vacancy. Closing date: September 27, 2025. Location: Wellington County including Kitchener, Waterloo, Cambridge and Guelph. May require travel to other programs as needed. Wages: LIUNA Union Wage Grid; $27.52-$29.84 per hour. Hours: Days (8am-4pm) with 1-3 weekend rotation; approx. 75 hours bi-weekly. Union: LIUNA.

Overview

St. Joseph’s Home Care (SJHC) is dedicated to providing quality and compassionate client‑centered care to individuals in their homes and/or alternative settings through our programs. Registered Practical Nurses enhance the life of their clients by providing a broad range of care needs within the policies and standards of practice prescribed by the College of Nurses of Ontario, contributing to healing and independence and the continual improvement of community health.

Core Duties and Responsibilities
  • Utilize the nursing process and standards of care (College of Nurses of Ontario).
  • Monitor and comply with DRN documentation.
  • Work independently in the community in the execution of daily tasks.
  • Complete point of care assessments and comprehensive nursing assessments (e.g., Mini‑mental Status, depression, pain, etc.).
  • Practice infection prevention and control measures.
  • Engage as a support network for peers also working in the community.
  • Provide the highest quality care and participate in quality processes as required.
  • Complete all necessary client assessments.
  • Implement and deliver client’s plan of care; recommend plan adjustments to Managers.
  • Coordinate client care with team members and managers; ensure appropriate caseload management and ongoing monitoring.
  • Collaborate respectfully with the client, family members, and other team members; engage other care multidisciplinary providers as necessary (e.g., Physicians, OT, PSW).
  • Engage with external stakeholders as necessary (e.g., hospital partners); advocate for clients/caregivers collaboratively.
  • Perform real‑time documentation; interpret information to support decisions and facilitate communications.
  • Utilize technology to document and communicate with team members and stakeholders.
  • Apply current knowledge and research to practice; share knowledge with others; act as preceptor, coach or mentor as requested.
  • Maintain current competencies and participate in continuing professional development; seek advanced or specialized nursing qualifications as needed.
  • Carry out self‑appraisals in accordance with the College of Nurses of Ontario Standards of Practice; participate in SJHC self‑evaluation and appraisal processes.
  • Participate in mandatory training and corporate quality initiatives; maintain privacy and confidentiality of all health records and related systems (including Procura and HPG).
  • Order nursing supplies and equipment as appropriate; support referral and intake processes; perform general administrative/non‑clinical duties as required.

Qualifications
  • Diploma in Practical Nursing from an accredited college, required.
  • IV medication administration education and/or experience, preferred.
  • Additional nursing specialty skills, preferred.
  • Current and unrestricted registration with the College of Nurses of Ontario, required.
  • Previous experience in a home or community nursing environment, preferred.
  • Previous experience with dementia care, required.
  • Experience in a unionized environment, an asset.
  • Previous experience as a Preceptor, preferred.
  • Basic Cardiac Life Support (BCLS) certification, required.
  • Experience using mobile tools/devices for work, preferred.
  • Experience with ESAS and/or PPS, required.
  • Chronic disease management or peritoneal dialysis experience, an asset.
  • Experience with complex clients, preferred.
  • Experience with Gentle Persuasion Approach or equivalent, an asset.
