Trillium Health Partners (THP) is one of the largest community-based acute care facilities in Canada. Responsibilities:

  • Assist home hemodialysis and Peritoneal patients with treatments within the home setting according to treatment plan while promoting quality of life for patients.
  • Complete and maintain related records and documentation.
  • Continuously observe patients and assist with necessary machine alarms; report status changes to the primary registered nurse and document.
  • Assist teams with necessary supplies to provide dialysis treatments.
  • Ensure correct use of equipment according to guidelines and protocols; report any equipment that is out of order and remove it from use.
  • Follow all infection control precautions and protocols.
  • Report any requests or complaints made by the patients and families to the Primary Home nurse or Home Transition Unit Coordinating Nurse (CN) or Peritoneal dialysis coordinating nurse.
  • Maintain confidentiality in all aspects of work.

Qualifications:

  • Diploma graduate of a recognized college or school of nursing or equivalent (i.e., certificate RPN with added courses in medication administration and health assessment).
  • Current certification or eligibility for certification of competence from the College of Nurses of Ontario.
  • Current BCLS certification.
  • Minimum of two years recent dialysis or Medical-surgical experience required in an acute care setting.
  • Knowledge of the Nephrology Nursing Standards and Practice Recommendations and Canadian Association of Nephrology Nurses and Technologists (CANNT) as they relate to hemodialysis including all procedures pertaining to patient preparation, treatment, and monitoring during and after the procedure.
  • A physical assessment course is required.
  • Demonstrates knowledge of care for the patient with end stage renal disease.
  • Recent experience with the renal patient population preferred.
  • Demonstrates knowledge of and experience with wound, skin, and foot care; the diabetic patient; venipuncture; routine practices of infection control; normal and abnormal blood values.
  • Demonstrates technical ability.
  • Previous experience in patient education programs.
  • Demonstrates excellence in communication skills and teamwork.
  • Demonstrates strong attention to patient and family-centred care and establishing therapeutic relationships.
  • Evidence of ongoing professional development preferred.
  • Assignments will include Community Home dialysis, Renal Care Centre satellite unit, Speakman dialysis care unit, CVH and Home Transition Unit.
  • A positive work and attendance record is essential.
  • Valid Driver's license in good standing with a reliable vehicle to travel within the THP community.
  • Must be a self-directed individual with well-developed organizational skills.
  • Ability to take initiative and work with minimal supervision.
  • Traveling required.
