Collingwood General and Marine Hospital (CGMH) is an 84 bed hospital located in Collingwood, Ontario (approximately 90 minutes north of Toronto) that serves more than 74,000 permanent residents and 3.5 Million annual visitors to the communities of Wasaga Beach, Collingwood, Clearview and the Blue Mountains. CGMH is an acute care hospital providing emergency care, diagnostic services including lab, imaging and cardio respiratory therapy, as well as two inpatient units (medicine and surgery). In addition, CGMH also provides care in specialty areas including obstetrics, orthopaedics, intensive care and surgery. The Hospital also provides out patient care including dialysis and a wide range of clinics including mental health and rehabilitation services. CGMH continues to provide care close to home for our community and plays a key role as an integrated orthopaedic centre for our region. The Opportunity

Participates as a team member in the promotion, assessment, provision and delivery of quality care through supportive, preventative, therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function. Provides direct care for individuals, group of individuals including family members and significant other whose outcomes of care are predictable. RPNs that provide aspect of care in situation in which the patient’s condition is complex and the outcomes of care are not predictable do so in collaboration with RNs and with other member of the health care team.

Qualifications

  • Current Certificate of Registration as a Registered Practical Nurse with the College of Nurses of Ontario
  • Possess current Basic Cardiac Life Support
  • Certified in physical assessment, asepsis.
  • Knowledge of principles of medical, surgical, geriatric, palliative, rehab (dependent on work area).
  • Successful completion of IV Certification
  • Excellent nursing assessment, communication, critical thinking, organization, documentation and problem solving skills
  • Strong conflict resolution skills when dealing with staff, supervisors, physicians, patients and family members.
  • Attention to detail and accuracy.
  • Excellent interpersonal, facilitation and communication skills.
  • Strong analytical and critical thinking skills.
  • Ability to exercise good judgement in recognizing situations that may require immediate attention of appropriate person/people.
  • Ability to provide educational opportunities for clients and family, other health care providers and health care students.
  • Knowledge of and adherence to Occupational Health and Safety and Infection Control Practices
  • Must strive to continuously provide a safe and supportive environment for patients, staff, physicians, and volunteers

Assessment

Duties and Responsibilities:

  • Assesses patients using a pre-established process
  • Exercises clinical judgement within the scope of practice.
  • Documents assessment according to the College of Nurses and CGMH Policies and Procedures.
  • Collects and interprets subjective and objective data to identify care needs.
  • Uses a variety of data collection and communication techniques and data sources.

Planning

  • Contributes to the plan of care and activities for the patient using a consultative collaborative process to include:
  • Care needs derived from assessment and input from the individual and other health care disciplines.
  • Nursing interventions for each care need.
  • Expected patient focused outcomes for each care need.
  • Evidence-based nursing actions.
  • Review of the plan each shift with revision of the plan according to ongoing assessment.
  • Participation in discharge planning.
  • Collaboration with other members of the team.
  • Documentation reflective of the plan of care.

Implementation

  • Administers patient care within scope of practice based on the established care plan.
  • Provides health teaching according to a pre-established plan.
  • Performs technical skill safely according to scope of practice, CGMH and unit specific Policies and Procedures.
  • Meets the Hospital and unit specific standards for certification.
  • Organizes and prioritizes care as mutually agreed with the patient and based on the nurse’s clinical judgement.
  • Demonstrates flexibility and adaptability.
  • Seeks help and guidance as required.
  • Documents all strategies and interventions.

Evaluation

  • Evaluates the patient response and outcomes using a collaborative and consultative approach.
  • Monitors patient responses and outcomes.
  • Contributes to the modification of the plan of care to reflect the patient’s changing needs.
  • Anticipates and/or responds to expected and unexpected outcomes.
  • Solicits assistance in response to unexpected outcomes.
  • Documents ongoing evaluation of patient responses and outcomes.

Coordination

  • Utilizes Organizational skills to facilitate care for assigned patients.
  • Works collaboratively with colleagues on other units to share resources, problem solve and plan for care provision.

Communication

  • Works with the patient, the health team and support services to provide professional service.
  • Accesses a variety of resources representative of the multi-disciplinary team within the organization and, where required, of the community and other health care agencies.
  • Reports changes in patient status to RN/MD.
  • Uses skills and knowledge to act as a client advocate.
  • Communicates in a manner that promotes respect for the individual.
  • Identifies issues/concerns with respect to compromises to client care, the Hospital’s Policies, Procedures and practices, etc. and initiates action to adjust the matter.
  • Reports maintenance issues/equipment repairs.

Hours of Work: Please be reminded that as a Hospital, in order to provide the necessary patient care, our operations run 24/7. Therefore, depending on the program for which you are applying, it may be an expectation that you are available to work weekdays and weekends, as well as all shifts - days, evenings, and nights.

Available on a predetermined basis for up to 22.5 hours per week (45/pay).

Wage: $36.777– $38.161
