Job summary

Southbridge Care Homes in Warkworth, Ontario, is seeking a Registered Practical Nurse for a part-time evening position (2:00pm–10:00pm) with rotational weekends. The role requires delivering high-quality resident care, adhering to nursing standards, and collaborating with care teams to support seniors in long-term care settings.

You will assess, plan, implement, and evaluate care plans, monitor residents’ health, and ensure accurate documentation.

Qualifications

• Current certificate of competence from the College of Nurses of Ontario is required.
• Registration or eligible for registration with the College of Nurses of Ontario.
• Comprehensive knowledge of nursing and health care practices, including infection control.
• Effective communication and ability to work independently and with teams.
• Negative vulnerable sector check completed within 6 months of hire date.
• Completion of a 2 step TB skin test within the last 12 months.

Responsibilities

• Provide resident care according to nursing standards, physician orders and policies, observe and report vital signs.
• Assess residents, develop and evaluate care plans, monitor health and refer as needed.
• Direct and supervise nursing staff, orient new personnel and participate in education.
• Ensure compliance with policies, procedures and regulatory standards.

Reporting to the Director of Care, the Registered Practical Nurse (RPN) will enhance the effective management and quality of nursing practice in keeping with the Standards of the College of Nurses of Ontario, Long Term Care Standards and Southbridge Care Homes Standards.

This is a Part-Time role with working hours from 2:00pm-10:00pm including a rotational weekend schedule, though extended hours may be required from time to time. The environment is fast-paced and deadline-driven, requiring high attention to detail and strong time management skills. The role includes regular use of computer systems and collaboration with cross-functional teams.

Key Responsibilities

• Ensure resident care is provide with established nursing standards and principles, physicians’ orders and administrative policies, as well as observe, report and record vital signs, symptoms and conditions of residents, and initiate remedial health measures where appropriate. Assure observance of required techniques in nursing care.
• Assess residents and develop, implement, review and evaluate resident care plans. Monitor residents’ health, notes changes in condition and assess need for referral to doctors; processes and implement physician orders.
• Organize and direct functions and activities related to provision of care to residents including restorative care and palliative care as required. Interprets and implements established policies, procedures, and regulations.
• Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in-service education. Monitor performance and conduct performance appraisal and refers problems to the Director of Care.

Knowledge And Skills Required

• Appropriate current certificate of competence from the College of Nurses of Ontario required.
• Registration or eligible for registration with the Registered Practical Nurses Association of Ontario.
• Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices.
• Effective human relation skills and demonstrated commitment to working with seniors.
• Effective communication skills and ability to work independently and collaboratively are essential.
• A negative vulnerable sector check completed within 6 months of hire date.
• Completion of a 2 Step TB Skin test within the last 12 months.
