Rightway Health is seeking a Clinical Guide Population Health RN to perform clinical triage, high-risk outreach, and care coordination for members. The role supports navigating and accessing high-quality care while reducing unnecessary visits.
The position is a Denver Hybrid role with remote Mondays, Wednesdays, and Fridays; in-office Tuesdays and Thursdays, shift 9:00 am–5:30 pm MST. Occasional holidays/weekends may be required.
Under the general supervision of the Sr. Clinical Manager, the Clinical Guide Population Health RN performs clinical triage, high-risk outreach, condition support, care planning, clinical education, and care coordination to support Rightway members in navigating, accessing, and best meeting their care needs. The Clinical Guide Population Health RN also serves as a trusted advisor for providing clinical guidance and directing to the most appropriate high-quality care providers with the objective of improving clinical outcomes while also reducing healthcare costs, e.g., minimizing ED or other unnecessary care visits while increasing utilization of more appropriate care settings, such as primary care doctors.

Compensation offered will be determined by geographic location, experience, and qualifications.
