The Registered Nurse - Transition Coordinator (TC) is a clinical consult-based position. TCs assess patients’ needs holistically, considering their life situation & determinants of health. Critical thinking & accurate interpretation of complex information are essential for clinical decision-making regarding continuing care options. The TC's role includes comprehensive assessment, planning & coordination of care, ongoing communication with healthcare providers, and liaising with community resources. Responsibilities include completing electronic needs assessments, prioritizing urgency, developing service plans, coordinating services, and supporting clients, families, & caregivers. The TC works within a multidisciplinary team and is responsible for referrals, documentation, participation in team meetings, evaluation, and professional development. This role may be situated in an acute care setting or involve placements at Westview, Leduc, or Devon facilities, depending on operational needs.
As a Registered Nurse (RN), you will provide nursing services to patients, families, communities, and populations, ensuring their safety and well-being. You will utilize nursing processes, critical thinking, problem-solving, and decision-making, while teaching, counseling, and advocating for patients and families. You will support an integrated, holistic approach to patient care, health promotion, and maintenance, reflecting AHS's shared values.
Provisional Permit Holders are paid $39.58 - $54.16/hour. Upon registration with the College of Registered Nurses of Alberta, pay adjusts to the applicable RN rate per AHS/UNA agreements.
Knowledge of computer applications such as Microsoft Word, Pathways, InterRAI Tools, Connect Care, Meditech, and ePeople is preferred and an asset.
