Job description
Position Summary

LNPLC seeks a dedicated Registered Nurse to act as a System Navigator to support individuals with their primary care needs, with a focus managing chronic conditions such as COPD and CHF. This position will also liaise with the Noojmawing Sookatagaing Ontario Health Team to assist in connecting patients from the Health Care Connect list to primary care in the region. This role involves case management, support, and guidance, helping patients navigate their primary care experience. The System Navigator will be a trusted point of contact, offering culturally-sensitive, personalized guidance, education, and improved access to health care services and other resources.

Key Responsibilities
  • Employ a person-centered approach in all interactions.
  • Coordinate care and connect patients with health care providers and community resources.
  • Develop and implement individualized care plans.
  • Assess patient needs and match them with appropriate health care services.
  • Facilitate continuity of care, including follow-up calls.
  • Facilitate referrals to health programs, maintaining clear communication between providers.
  • Encourage and support self-management of chronic conditions.
  • Build and maintain relationships with community partners.
  • Participate in program research, data collection, and evaluation.
Education, Qualifications, and Skills
  • Current registration with the College of Nurses in Ontario
  • Minimum of a Diploma as an RN from an approved institute, preference given to a BScN from an approved University
  • 3-5 years experience working as a RN
  • Experience with electronic documentation
  • Demonstrated ability to work independently and as a member of a multi-disciplinary health services team
  • Ability to work with diverse communities with respect and cultural understanding.
  • Experience in building and maintaining relationships with community partners.
  • Excellent interpersonal, communication.
  • Strong time management and prioritization skills.
  • Proficiency in computer and word processing skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Current BCLS.
Additional Job Requirements
  • Satisfactory criminal records check, including vulnerable sector screening.
  • Valid Class “G” Driver’s License and access to a personal vehicle.
  • Up to date on vaccinations (including MMR and Covid) – must provide immunizations & Covid vaccination record
