Job description

St. Joseph’s Home Care (SJHC) is dedicated to providing quality, compassionate, and client-centered care to individuals in their homes and/or alternative settings through visiting, shift, or the Integrated Comprehensive Care programs. Registered Nurses enhance the lives of their clients by providing a broad range of care needs within the framework of SJHC’s policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario. Through their work, Registered Nurses contribute to healing, independence, and the overall health of the community.

CORE DUTIES AND RESPONSIBILITIES
  1. Utilize the nursing process effectively to provide high-quality Wound and Ostomy Care for clients and/or caregivers.
  2. Collaborate with the interdisciplinary health team to deliver cost-effective, high-quality wound care using best practices.
  3. Apply current knowledge and research of wound care protocols to practice.
  4. Develop, in consultation with the physician, CCAC Case Manager, ICCP coordinators, and Primary Nurse, a plan of care for patients requiring wound care in the community.
  5. Initiate and modify the nursing care plan in consultation with the client and/or caregiver.
  6. Promote client and caregiver independence, considering their needs, strengths, and weaknesses.
  7. Utilize available resources and facilitate problem solving, decision-making, and access to appropriate resources for clients and caregivers.
  8. Participate in the evaluation and refinement of the Wound Care Protocol and overall program.
  9. Educate other SJHC team members on the protocol and product usage, guiding best treatment choices.
  10. Maintain regular communication with supervisors to review issues and exchange feedback.
  11. Participate in bi-weekly nursing case rounds.

KEY EDUCATION AND EXPERIENCE
  1. Bachelor of Science Degree in Nursing from an accredited university.
  2. Current, unrestricted Registered Nurse license and good standing with the Ontario College of Nurses.
  3. Proficiency in MS Office, including Excel.
  4. 1-3 years of experience in home, community, acute, or long-term care nursing.
  5. Familiarity with Procura or similar scheduling software is an asset.
  6. Certification as a wound care nurse or completion of equivalent courses (e.g., IIWCC, SWAN) is recommended.
  7. Experience in complex wound care is required.
  8. Experience in ostomy and/or continence care is preferred.
  9. Experience with palliative care is preferred.
  10. Proven excellence in direct client care.
  11. Excellent communication and interpersonal skills.
  12. Ability to travel daily within a defined geographical area.
