The Calgary West Central Primary Care Network (CWC PCN) is a non-profit organization committed to a healthier future. We work alongside more than 475 family doctors to provide support for continuous health care.

We are seeking a Registered Nurse (RN) to provide comprehensive nursing services to patients who are physiologically aged and/or elderly. In this role, you will work to your full scope of practice delivering evidence-based care, assessments, and interventions with an emphasis on providing comprehensive care, education, and guidance to the elderly and their families. You will facilitate the transition of a patient’s care between and across services and providers, including the transitions between acute, primary, and community-based services.

In this hybrid position, you will conduct home visits, meet with patients in-person at our Primary Care Centre, and work remotely from your home office. Therefore, the successful candidate must be located within Calgary or the surrounding area (e.g., Cochrane, Airdrie, Chestermere, etc.).

This is a temporary contract (maternity leave cover), and the contract is expected to end on February 13th, 2026.

Responsibilities

Clinical expertise

  • Work collaboratively within a multidisciplinary team to enhance the delivery of primary care services to patients 65 years of age and above, and those who are over 55 years of age who are physiologically aged.
  • Perform and document a comprehensive history and in-depth assessment of the health status including physiological, psychosocial, economic, learning, cultural, and spiritual factors affecting health and well-being. Initiate and implement management strategies and provide appropriate evaluation, follow-up, and modifications to care plans.
  • Integrate scientific theories and current research and evidence from nursing and allied healthcare professions. Identify and articulate the rationale for selecting interventions, and implement innovative approaches to support the health, learning, and transitional needs of the individual, family, and community.
  • Provide chronic disease management and mental health services within the context of geriatric care, incorporating monitoring progress and response to treatment, and promoting the preventive aspects of patient care. Provide necessary interventions, within the scope of practice.
  • Initiate appropriate investigations and community referrals to broaden the basis of support for patient, family, and caregiver.
  • Conduct patient visits in a variety of settings to best address patient need. These include virtual visits, clinic settings, and community/home visits.

Community safety

  • Promote a healthy/safe practice environment, recognize safety issues and report appropriately, and participate in correction and prevention action plans.
  • Conduct and document environmental risk assessments prior to community visits to ensure patient and staff safety.
  • Maintain vigilance during community visits to ensure patient and staff safety. Utilize de-escalation training and techniques when required.

Requirements

Education: A bachelor’s degree in nursing from an accredited post-secondary healthcare program with an equivalent combination of training and experience in geriatrics is required. Equivalencies of education may be considered.

Experience: Two - five years’ experience with broad clinical nursing practice is required. Two-five years’ experience with the following is an asset:

  • Nursing experience within gerontology and chronic disease management.
  • Experience working in a primary care environment.

Equivalencies of experience may be considered.

Registration: Active registration with the College of Registered Nurses of Alberta (CRNA) is required.

Certification: The Canadian Nurses Association professional certification related to Geriatrics, GNC(C) or willingness to take is required.

CPR: Current BLS Level C CPR/AED issued by the Heart and Stroke Foundation.

Background check: A clear Criminal Background check with Vulnerable Sector check is required to start.

Driver’s license: A valid Alberta driver’s license and reliable vehicle is required.
