Job summary

North Haida Gwaii Hospital, part of Northern Health, seeks a Registered Nurse/LPN Team Lead to supervise Home Support Services, coordinate client care plans, and mentor Community Health Workers.

Under the supervision of the manager, you will train staff, ensure safe, ethical care across settings including homes and clinics, and collaborate with an interprofessional team to meet patient needs.

Qualifications

  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
  • Two (2) years of recent, related experience including one (1) year in a supervisory role; or an equivalent combination of training and experience.
  • Current valid BC Driver's License.

Responsibilities

  • Supervise Community Health Workers and oversee the implementation of client care plans.
  • Provide training and evaluation of personal care techniques.
  • Advocate for patients and families and work as part of an interprofessional team to deliver safe, ethical care.
  • Coordinate Home Support Services across settings such as physician practices, homes, schools, and communities.

Position Summary

Do you have a passion for nursing? We have the position for you! We are seeking a motivated Registered Nurse with a dedication for delivering compassionate patient care to join our team at North Haida Gwaii Hospital.

In accordance with the established mission, vision, values, and strategic priorities of Northern Health, and under the general supervision of Manager or designate, the Licensed Practical Nurse - Home Support Team Lead practices in accordance with the standards of professional practice and code of ethics as outline by the BC College of Nurses and Midwives. Coordinates Home Support Services by supervising Community Health Workers, overseeing the implementation of client care plans, providing training and evaluation of personal care techniques, provides advocacy services, and also functions as a member of the interprofessional team and applies best practice to provide competent, safe, and ethical care for people and their families in settings such as physician practices, homes, schools, and communities.

Primary Care Networks are partnerships between the Divisions of Family Practice representing BC's family physicians, the regional health authorities, the First Nations Health Authority (FNHA) and local community partners, along with other community providers such as nurse practitioners. All partners are working together in their local communities and collectively to redesign B.C.'s primary and community care system so that it works better for patients and health-care providers.

Shift Rotation/Hours of work

Monday, Tuesday & Thursday, Friday, Days, 07:30 to 15:30

Qualfications
  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
  • Two (2) years of recent, related experience including one (1) year in a supervisory role; or an equivalent combination of training and experience.
  • Current valid BC Driver's License.
Skills and Abilities
  • Ability to supervise others.
  • Demonstrated knowledge and skills within a specific clinical specialty or patient population dependent on the nature of the caseload (e.g. acute care, gerontology, palliative, etc.).
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to establish and maintain rapport with clients.
  • Ability to work independently and collaboratively with others.
  • Ability to observe and recognize changes in clients.
  • Ability to plan, organize and prioritize; and ability to implement a plan of care.
  • Ability to operate related equipment including proficiency in relevant computer applications.
  • Physical ability to carry out the duties of the position.
  • Use of own vehicle.
