WellFort Community Health Services is seeking a Registered Nurse (RN) for the Diabetes Education Program (DEP) in Brampton, ON. The role provides diabetes education, assessment, counseling, and self-management support for adults with Type 2 diabetes and pre-diabetes.
The RN works within an interdisciplinary team, collaborating with physicians, dietitians, and community partners to deliver high-quality care. CDE designation is mandatory and CNO registration is required.
Diabetes Education Program Full Time Brampton, ON $34.88 - $41.16 CAD per hour
Applicants must hold a current Certified Diabetes Educator (CDE) designation to be considered for this position.

Position Summary
Reporting to the Manager, Diabetes Education Program, the Registered Nurse (RN) is a key member of the interdisciplinary Diabetes Education Program (DEP) team. The RN provides comprehensive diabetes education, assessment, counseling, and self-management support to adults living with Type 2 diabetes, pre-diabetes, and individuals at risk of developing diabetes.
Working within a client-centered, culturally responsive, and evidence-informed framework, the RN supports clients in achieving optimal health outcomes through individual consultations, group education, chronic disease management, health promotion activities, and community outreach initiatives. The RN collaborates closely with physicians, nurse practitioners, registered dietitians, and community partners to deliver high-quality diabetes care and education.
