Reporting to the Director of Care (DOC), the Licensed Practical Nurse (LPN) is responsible for providing direct resident care, medication administration, and documenting care in compliance with regulatory and company health and wellness standards, policies, and procedures.

Key Responsibilities

  • Complete and ensure residents’ care assessments, service plans, and documentation are up to date; communicate changes to DOC for billing purposes.
  • Assist with scheduling and participate in residents’ care conferences.
  • Work closely with DOC to facilitate residents’ move-in and transfers.
  • Order, safely store, and administer drugs and narcotics in compliance with all regulatory requirements and company policies.
  • Facilitate and provide resident and family education.
  • Supervise resident self-medication assessments and audits.
  • Supervise and delegate care and medication responsibilities to Unregulated Care Providers according to regulations and policies.
  • Plan, coordinate, manage, and evaluate residents’ care plans in collaboration with residents, decision-makers, physicians, and the interdisciplinary team.
  • Coordinate nursing team’s resident care assignments for efficient and timely service delivery.
  • Respond to and manage significant changes in residents’ conditions and medical emergencies.
  • Act as supervisor and coordinate emergency responses in the absence of DOC and Executive Director.
  • Address resident / family concerns, ensuring appropriate actions are taken and concerns are communicated to the DOC / Executive Director and other departments.
  • Assist in coordinating, implementing, and monitoring Infection Prevention and Control, Health and Safety, and Clinical Programs and Quality Plans for the Nursing Department.
  • Coordinate and supervise scheduling and assignments of nursing students from approved educational institutions.
  • Uphold and promote the organization’s values and philosophy, especially regarding ethics, morality, and integrity.
  • Complete all other tasks as assigned.

Required Education, Credentials, and Experience

  • Current registration as a Registered Nurse in the applicable Province / State.
  • Experience working with seniors and unregulated care providers.
  • Education and experience in gerontology, rehabilitation, community, complex, or long-term care are assets.
  • Strong written and oral communication skills, critical thinking, care coordination, time management, and customer focus.
  • Demonstrated clinical knowledge and ability to delegate to unregulated care providers.
  • Maintain competencies as per the governing College standards.
  • Proficiency in computer use and Microsoft Office applications.
  • Current (within 6 months) Vulnerable Position / Sector Screening (VPS).
