Registered Nurse/Registered Practical Nurse (0.6 FTE, contract)

The Thames Valley Family Health Team is one of Ontario's largest family health teams, with over 120 physicians across 19 sites in London/Middlesex, Oxford, and Elgin counties. We are committed to providing comprehensive, collaborative primary health care to more than 160,000 patients. Our team includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists, and other health care professionals working together to deliver patient-centered care. Skills & Qualifications
  • Current registration with the College of Nurses of Ontario
  • Interest and experience in program development and group program delivery
  • Ability to provide episodic care under a medical order or directive
  • Knowledge of working collaboratively with community resources
  • Understanding of Chronic Disease Management and Patient Self-Management Models
  • Proficiency in computer use and experience with Electronic Medical Records (EMR)
  • Strong interpersonal skills and ability to work effectively in a team
Salary

Dependent on RN or RPN experience. RPN salary range: $55,325 - $67,469. RN salary range: $67,887 - $82,789.

Interested candidates should submit a detailed resume by May 21, 2025, at 4 pm. Successful candidates will undergo a criminal background check and medical screening. Applicants must have a Social Insurance Number, valid work permit if applicable, and proof of COVID-19 vaccination (two doses, second dose received more than 14 days ago). Internal candidates will be given preference. Only those under consideration will be contacted for an interview.
