The role involves working with an inter-professional team to provide primary care services in various settings, including UPCC, Patient Medical Homes, and Family Physician Clinics.
The RN - Primary Care practices according to standards set by the BC College of Nurses and Midwives, applying best practices to deliver safe, competent, and ethical care. Responsibilities include initial assessments, health promotion, health coaching, client education, and facilitating care transitions to promote health and prevent hospital admissions.
Qualifications include registration with BC College of Nurses and Midwives, two years of recent clinical experience in primary care or related settings, a valid BC driver's license, and BLS Level C certification.
Skills required encompass cultural humility, trauma-informed care, effective communication, teamwork, data analysis, health education, time management, and knowledge of primary care practices.
Travel may be required, with transportation arrangements meeting operational needs.
