Job summary

A large faith-based health organization in Vancouver is seeking a Staff Nurse to join their interdisciplinary team. The Staff Nurse will assess patients' needs, develop individualized care plans, and provide comprehensive support. Job description

The salary range for this position is CAD $41.42/Hr. - CAD $55.91/Hr.

As a Staff Nurse, you'll be at the forefront of patient care, working collaboratively within an interdisciplinary team to ensure patients/residents receive comprehensive support.

Responsibilities
  • Assess patients' physical, social, spiritual, and psychological needs.
  • Develop, implement, and evaluate individualized care plans.
  • Incorporate teaching into care plans to educate patients and families.
  • Make referrals to resources as needed.
  • Support fellow healthcare professionals.

Education
  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.

Locations
  • St. Paul’s Hospital (10D) *Acute Palliative care focuses more on acute pain and symptom management needs. Many patients may go home after their time in palliative care or transfer to hospice or long-term care.
  • May’s Place (6 beds) *Sub-Acute Downtown Eastside, where people have problems with mental health and substance abuse.
  • St. John’s Hospice (14 beds) *Sub-Acute Hospice care is intended for those who are going through their last days of life and are receiving care for end of life. The first community hospice on Vancouver's West Side and is situated in a beautiful location on the University of British Columbia's campus.
Your Day to Day
  • Conduct thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers
  • Develop, implement, and evaluate nursing care plans based on evidence-based practices, prioritizing patient/resident needs.
  • Adhere to established policies, procedures, and PHC standards while providing high-quality nursing care
  • Monitor patient/resident progress, evaluate outcomes, and adjust care plans as needed for optimal results
  • Maintain accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems
  • Identify and address patient/resident and family learning needs, providing education and referrals as necessary
  • Provide direction to staff, promote teamwork, and share clinical knowledge to enhance patient care
  • Support nursing research initiatives and contribute to the improvement of patient/resident care policies and procedures
  • Advocate for patients/residents' rights and dignity, facilitate discharge plans, and collaborate with families and community resources
  • Perform additional duties as assigned to support the healthcare team
