Job summary

Trillium Health Partners invites applications for a Registered Nurse at McCall Centre Rehab (2nd floor) on the Queensway Health Centre campus. This temporary full-time position supports Primary, Rehab, CCC & Seniors, with 12-hour days and nights, seven days a week including holidays.

You will provide nursing assessment, develop care plans, educate patients and families, and contribute to discharge planning.

Qualifications

  • Graduate of a recognized School of Nursing.
  • Bachelor’s degree in Nursing is required.
  • Current CN Ontario certification or eligible for certification is required.
  • Current BCLS required.
  • Recent inpatient medical population experience.
  • Recent rehab population and complex care knowledge.
  • Knowledge of geriatric rehabilitative nursing concepts.
  • Ability to educate patients and families.
  • Demonstrates patient- and family-centered care.
  • Demonstrates critical thinking and team leadership.

Responsibilities

  • Responsible and accountable for nursing care decisions and management for patients.
  • Provides nursing assessment and contributes to care plan development.
  • Delivers education and support addressing medical, cognitive, emotional and self-care needs.

About McCall Centre

McCall Centre is a 120-bed chronic care facility situated on the Queensway Health Centre campus. Though a chronic care facility, most of our patients are transitional in nature for reintegration back into the community. Our specialized interprofessional teams will work with families to support patients’ plan of care and goals with compassion and excellence.

Position Summary
  • Responsible and accountable for decisions and actions of the provision and management of nursing care for patients;
  • Provides nursing assessment, contributes to the care plan development, provides goal oriented treatment/interventions, patient and family education and support to address medical, cognitive, emotional and self-care needs;
  • Contributes to the development of a realistic discharge plan
  • Acts as a coach/mentor/consultant and uses sound critical thinking skills which are key to achieving patient outcomes and facilitating team functions;
  • Participates in program planning, continuous quality improvement and evaluation.
  • Willingness to take on unit clinical leadership during the shift
Qualifications
  • Graduate of a recognized School of Nursing
  • Baccalaureate degree required
  • Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required
  • Current BCLS required
  • Recent experience with inpatient medical population required
  • Recent experience with inpatient rehab population and complex care needs population required
  • Recent knowledge of the philosophy and concepts of geriatric rehabilitative nursing practice
  • Demonstrates ability to provide patients and families with educational resources
  • Demonstrates patient and family-centered care
  • Demonstrates knowledge and ability to establish a therapeutic relationship
  • Evidence of on-going professional development
  • Demonstrates critical thinking, problem solving and leadership within a team
  • Must be reliable, flexible, creative, self-directed to work effectively and collaboratively in a high performance interprofessional team
  • Satisfactory attendance and performance required
