Job Description

Position Overview:

  • Working as a collaborative member of the inter-professional team, the Registered Nurse (RN) functions to support the care needs of our community who do not have a primary care provider. The RN will assist in the development of comprehensive care plans, care navigation and facilitate clinical decisions to appropriately manage acute/chronic illness and promote health and wellness for children, adults, and seniors. The IPCT RN will:
    • Practice in accordance with the Regulated Health Professions Act, the Practice Standards set by the College of Nurses of Ontario and standards set out for Registered Nurses
    • Lead the intake process to ensure referrals are mapped to appropriate care pathway(s)
    • Conduct physical assessments, vital sign monitoring and complete point of care testing as required
    • Administer medications and treatments as prescribed by physician/nurse practitioner.
    • Perform clinical procedures according to nursing practice standards
    • Contribute to the development and monitoring of patient care plans
    • Provide health education to patients and families with a focus on health promotion, disease prevention, and chronic disease self-management.
    • Assist with care navigation to streamline access.
    • Maintain ongoing awareness of change in technology, treatment of illness and nursing practices
    • Have deep understanding of the social, economic, and cultural conditions of the immediate community and neighbourhoods
    • Support evaluation and quality improvement activities to assess impact and outcomes and determine strengths, opportunities, and future directions of the IPCT services.
Requirements

  • Current registration from the College of Nurses of Ontario and a member in good standing
  • Minimum of 3-5 years recent experience in primary care or community setting
  • Strong knowledge of chronic disease self-management,
  • Experience with care navigation and referral pathway development
  • Demonstrate thorough knowledge of home and community-based care practices, community resources and ability to provide linkage to the appropriate services necessary to best serve the person.
  • A proven ability to liaise with referral sources and other community agencies to promote effective inter-collaborative communication and positive relations among care partners.
  • Ability to prioritize and effectively manage a diverse caseload, utilizing exceptional time management skills.
  • Excellent communication skills both written and verbal.
  • Advanced problem-solving, decision making and critical thinking skills.
  • Demonstrates initiative and the ability to function both independently and as an interdisciplinary care team member.
  • Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
  • Utilize protective equipment provided and identify and reports hazards.
  • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community.
  • Excellent work performance and attendance
