The Opportunity

Reporting to the Manager of Hematology/Oncology/Nephrology and PACT, the Family Care Coordinator Registered Nurse (FCC RN) is a member of an interdisciplinary care team, providing holistic, comprehensive, evidence-based care to meet the needs of Nephrology patients and families. The FCC RN provides leadership in nursing assessment and the planning, implementation, and evaluation of patient/family care. The FCC RN provides care in collaboration with the patient and family, other nurses, and health team members within the IWK and external organizations throughout all stages of health and the complexities of illness. The FCC RN builds professional relationships with other nurses and health team members to support the work of a strong interprofessional team and the provision of quality patient/family care. The FCC RN ensures the goals and needs of the patients and families are prioritized and individualized, using nursing knowledge, critical thinking, and clinical judgment.
The Nephrology FCC RN provides patient care primarily within the Nephrology Clinic, both in joint appointments with physicians and nursing-only visits. The FCC RN supports the coordination of care for patients followed within the Nephrology Clinic, the Renal Dialysis Unit, and 6Link. The FCC RN will provide care and teaching for PD, hemodialysis, renal transplants, and other nephrology conditions as needed. The FCC RN will be responsible for monitoring the patient’s ongoing needs and information, as well as completing assessments in person and virtually (including phone follow-up). The FCC will maintain competencies in all areas of nephrology pediatric patient care and will complete required shifts within RDU to do so.
The RN practices according to the Canadian Nurses Association Code of Ethics for Registered Nurses, the College of Registered Nurses of Nova Scotia Standards of Nursing Practice, specialty standards as applicable, and the policies and procedures of the IWK. FCC RN will be responsible and accountable for establishing and maintaining a leadership role as the liaison person between the Nephrology Service, other Health Centre services, and community resources in the delivery of family-centered patient care consistent with the objectives and vision of the IWK Health Centre.

Hours of Work

Monday to Friday; 7.5 hour shifts; 0730-1530, as well as standby between 1500-0700 evenings and weekends as required.

Your Qualifications

• Bachelor's Degree in Nursing required.
• Registered, or eligible to be registered, with the Nova Scotia College of Nursing (NSCN) is required.
• Minimum three (3) years of recent pediatric nephrology experience required.
• Family Care Coordination experience preferred.
• Demonstrated competency in Hemodialysis, Apheresis, and Peritoneal Dialysis, and the ability to maintain those competencies required.
• Previous experience in patient/ staff education required.
• Current successful completion of CPR Level-C Healthcare Provider within the last 12 months is required.
• Meticulous attention to detail and follow through with demonstrated experience in organization and coordination of care for patients/ families, including experience with transition of care, discharge planning, and inter-agency coordination required.
• Demonstrated leadership, communication, and interpersonal skills, with an ability to interact both with interdisciplinary team members and with community partners in health.
• Demonstrated ability to function in and manage multiple priorities within complex systems is required.
• Flexibility required for occasional evening or weekend calls as needed.
• Certification in specialty practice (eg, CPON/CPHON, Nephrology certification) preferred.
• Competency in other languages is an asset.
