We are looking to hire Registered Nurses (RNs) to join our Transitions team - this is an exciting opportunity to be at the front-line of health care innovation in Canada.

Full-time Salaried - 37.5 hours per week

Sign On Bonus - $5000

POSITION SUMMARY :

We are seeking a dynamic and passionate Registered Nurse (RN) who wants to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You will work in an environment that emphasizes professional autonomy within a self-directed team, providing services to a community-based client roster. You will collaborate on care planning focusing on client-centered goals across physical, cognitive, social, emotional, and spiritual domains, creating holistic care plans. Each client is assigned a primary care clinician who leads care planning and facilitates interdisciplinary team efforts. As part of the SE Transitions team, you will be accountable for the care and outcomes of your clients.

Geography :

North to Hastings, south to Wellington, east to Brighton, and west to Tyendinaga.

Responsibilities :

  • Provide comprehensive care within an interdisciplinary team.
  • Serve as the primary contact for assigned clients.
  • Conduct assessments and develop tailored care plans.
  • Lead care planning and coordinate interdisciplinary efforts.
  • Facilitate team communication and discharge planning.
  • Organize family and caregiver meetings.
  • Support clients through care transitions and connect them with community resources.
  • Assist clients in developing self-care strategies and health promotion.

Requirements :

  • Registered with the College of Nurses of Ontario.
  • Minimum two years of nursing experience.
  • Experience in home / community care, acute care, chronic disease management, palliative care, or mental health.
  • Ability to work independently within RN scope of practice.
  • Community experience is an asset.
  • Demonstrated leadership and clinical skills.
  • Excellent assessment, communication, and organizational skills.
  • Valid driver’s license and ability to travel.
  • Membership in RNAO preferred.
