Our client is seeking experienced Registered Nurses to join their team at their care home. As an RN, you will provide clinical leadership and advanced nursing care to residents, oversee the nursing team, and ensure the delivery of high-quality, person-centered care in an environment dedicated to excellence and dignity.
Key Responsibilities
Clinical Leadership & Assessment
- Conduct comprehensive nursing assessments and develop individualized care plans
- Perform complex clinical procedures and advanced wound care management
- Oversee medication administration and narcotic control programs
- Lead admission, discharge, and transfer processes
- Collaborate with physicians to optimize resident care outcomes
- Supervise and mentor RPNs and Personal Support Workers
- Conduct performance evaluations and provide constructive feedback
- Coordinate staff assignments and ensure appropriate skill mix
- Lead shift huddles and communicate care priorities
- Foster a positive team culture focused on resident-centered care
Quality & Risk Management
- Monitor and ensure compliance with nursing standards and regulations
- Investigate and document incidents, implementing corrective actions
- Participate in quality improvement initiatives and audits
- Manage complex resident and family concerns
- Ensure adherence to infection prevention and control protocols
Care Coordination & Communication
- Lead interdisciplinary care conferences and team meetings
- Communicate effectively with physicians, families, and external providers
- Provide education and support to residents and families
- Document comprehensively in electronic health records
- Act as resident advocate and liaison with community resources
Required Qualifications
- Current registration with the College of Nurses of Ontario as an RN
- Bachelor of Science in Nursing (BScN) preferred
- Minimum two years of nursing experience, preferably in long-term care
- Valid CPR certification (HCP level)
- Strong leadership and clinical decision-making skills
- Proficiency with electronic documentation and health information systems
- Clear criminal reference check and vulnerable sector screening
Preferred Qualifications
- Additional certifications in gerontology, wound care, or palliative care
- Previous supervisory or charge nurse experience
- Knowledge of RAI-MDS and long-term care regulations
- Experience with dementia care best practices
- Project management or quality improvement experience
- Second language skills an asset
