Located in the Waterloo Region, we provide comprehensive primary care services to more than 30,000 patients. As a Registered Nurse (RN) with New Vision Family Health Team, you will provide comprehensive primary care nursing services to support the health and well‑being of patients across the lifespan. Working both independently and collaboratively with physicians, nurses, allied health professionals, and administrative staff, you will contribute to the delivery of effective, timely, and patient‑centred care while supporting patients in achieving their health goals. The RN will also play an integral role in supporting the team’s Diabetes and Hypertension Program, as well as other programs, services, and initiatives offered by the Family Health Team, helping to promote chronic disease management, prevention, health education, and overall patient wellness.
