Job summary

ParaMed Home Health Care in Hamilton, Ontario is seeking a dedicated Registered Nurse for community RN visiting nursing. You will provide patient‑centered care in clients' homes, with pediatrics and palliative experience preferred, and coordinate plans to keep patients comfortable at home.

In this full‑time role, you’ll assess needs, execute and monitor care plans, document encounters, and travel within the service area.

Qualifications

• Must be registered with the CNO as a Registered Nurse in good standing.
• Valid driver’s license and access to a vehicle.
• Experience delivering community care is an asset.
• Reliable and flexible with willingness to travel within designated area.
• On-call coverage, including after hours to ensure patient safety.

Responsibilities

• Using appropriate assessment tools to assess patients’ needs and monitor care plans.
• Execute and monitor care plans and update patient records.
• Engage patients using a collaborative approach by building rapport and trust.
• Travel to patient residences within a designated area to provide home care.
• Maintain and update professional patient records related to patient encounters and plans.

Job DescriptionIn this community RN Visiting Nursing role, as a Registered Practical Nurse (RPN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of palliative care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.
