Job summary

Taddle Creek FHT is seeking a Registered Dietitian to deliver clinical care and education through the Diabetes Education Program (DEP). The RD will assess, plan, and implement diabetes self-management education, and work within an interprofessional team to support patients and families in Toronto.

This permanent, full-time role is on-site at 790 Bay St, with 4 days on site and 1 day remote, 40 hours weekly.

Qualifications

• RD with College of Dietitians of Ontario.
• Bachelor of Science in Nutrition or equivalent required.
• CDE preferred or willingness to obtain.

Responsibilities

Job Description – Registered Dietitian, Diabetes Education Program (DEP)

Position Summary

Taddle Creek Diabetes DEP is a part of Taddle Creek Family Health Team (TC FHT). The DEP serves both TC FHT patients and patients/physicians in the community. Our goal is to provide people living with diabetes, or at risk of developing diabetes and their families, the knowledge and skills needed to self-manage.

As a member of the DEP the Registered Dietitian (RD) provides clinical care and education to people living with and at risk of developing diabetes. The RD participates in the development, implementation, monitoring, and evaluation of the DEP.

The RD has a sound knowledge base regarding the prevention, care and management of diabetes and practices within an interdisciplinary model. The RD utilizes self-management principles and practices patient-centered care. The RD acts as an expert in the areas of medical nutrition therapy and lifestyle changes for the management and prevention of diabetes. The RD is responsible for providing individual counseling and facilitating group education classes. It will be important that the RD works collaboratively with the DEP, as well as with patients and their families.

All performance and conduct of duties shall be carried out in accordance with TC FHT policies/procedures, professional practice standards and required legislation (e.g. Diabetes Canada, Regulated Health Professions Act).

Roles and Responsibilities

Patient Care

• Caseload: aim is to see 7-8 patients per day
• Practices in accordance with a patient self-management model to assist people living with diabetes to achieve their optimum level of health and quality of life
• Partners with patients and their families to enable them to design and manage their own individualized diabetes protocols/plans (e.g. establishes goals with patients)
• Collaborates directly with other DEP clinicians to form comprehensive care plans with the patient
• Maintains open communication and works closely with FHT and community primary care providers (PCP) on patient care issues
• Provides blood glucose monitoring training and pattern management guidance to patients
• Provides education around insulin initiation, titration, and other injectable therapies as prescribed by PCP
• Suggests medication adjustments to achieve optimal glycemic control and recognizes inappropriate medication orders
• Assesses patient educational needs by identifying physical, social and or emotional needs that may influence the education process and the long-term outcomes
• Evaluates patient comprehension and progress of their care plan by reviewing clinical and learning outcomes
• Accurately and professionally completes documentation in a timely manner using the DEP Custom Form in EMR
• Recommends appropriate follow up and provides community resources for patients to access
• Networks with RNs to create partnerships and receive support within the diabetes education community

Education/Group Counseling

• Plans, implements and evaluates patient and clinician group education
• Facilitates various group education classes for patients and clinicians
• Assists patients/FHT health professionals to use diabetic resources
• Prepares written diabetes educational pieces (for FHT newsletter or bulletin board/displays)
• Maintains own academic qualifications and participates in self‑directed learning to ensure their practice remains relevant (by attending professional conferences, e‑learning and journal reviews)

Skills, Knowledge Requirements

• Ability to mentor clinicians, and students within your scope of practice
• Strong clinical skills in diabetes self‑management education
• Excellent organizational skills (e.g flexible, good time mgt)

Equipment/Computer Skills

• Familiarity with a Mac operating system environment
• Willingness to learn electronic medical record keeping/scheduling
• Computer, printer, scanner, fax, photocopier, telephone, AV equipment (video projection), and a variety of teaching aids
• Various diagnostic and medical equipment

Language Skills

• English required; French not required but would be an asset
• Ability to communicate in another language an asset

Education and Specific Job Requirements

• Baccalaureate of Science in Nutrition or equivalent required
• Registered Dietitian with the College of Dietitians of Ontario required
• Membership with Dietitians of Canada Association, preferred
• Certified Diabetes Educator (CDE) with the Canadian Diabetes Educator Certification Board (CDECB) required (or willingness to obtain)
• Excellent assessment, planning and organizational skills
• Demonstrated proficiency in adult education and teaching
• Demonstrated proficiency in diabetes nutritional assessment, management and education

Relevant Experience

• Minimum 2 years of clinical experience as a Registered Dietitian, preferably in communitybased health care
• Minimum 1 years of health care teaching experience
• Minimum 1 year experience applying advanced diabetes knowledge, physical assessment skills, diagnostic intervention and self‑care management
• Program planning/evaluation experience (demonstrated ability to plan/implement/evaluate)
• Experience,
• o Providing specialized knowledge through the process of consultation at the patient, program, organization and external agency level
  • Designing, implementing and evaluating policy and best practice guidelines pertaining to diabetes management
  • Developing strategies to improve patient adherence and to improve chronic disease self‑management outcomes
  • Leading and facilitating the development of basic and advanced education programs
  • With insulin education, glucose pattern management and insulin pump therapy
  • With insulin injection devices, variety of home blood glucose monitoring devices
• Experience in the area of chronic disease self‑management and prevention
• Experience mentoring dietetic interns/ other health care providers
• Experience with Practice Solutions Suite (preferred) or other electronic medical record

Reporting Structure

The Registered Dietitian, Diabetes Education Program (DEP) reports to the DEP Team Lead for day‑to‑day operations of the DEP. The FHT Executive Director will provide professional guidance and support as needed.

Hours of Work: 40 hours per week, Mon‑Fri 8am‑4pm or 8:30am‑4:30pm, corresponding to the hours of the Diabetes Education Program. Some evenings and weekends may apply. Note: Expectation is for this position to be on site 80% (4 days a week), 1 day work from home (this is subject to change at any time by the organization)

Status: Permanent/Full Time

Pay Range: $70,590 - $77,825

Benefits: 22.5% (used for: statutory deductions, group insurance plan & Hospitals of Ontario Pension Plan - HOOPP)

Annual Vacation Days: 3 weeks (4 weeks after year 1)
