An established industry player is seeking a dedicated Patient Coordinator to enhance community healthcare delivery. This role emphasizes collaboration with healthcare services to provide immediate medical supervision and treatment outside traditional hospital settings. As a vital component of the Mobile Integrated Healthcare Coordination Center, you will guide Community Paramedics and EMS crews, ensuring patients receive timely care and resources. Your Opportunity:
The Patient Coordinator (PC) is a trained Community Paramedic (CP) with additional training to work within the Mobile Integrated Healthcare- Coordination Center (M.I.H.C.C.). The PC is responsible for tasks related to M.I.H.C.C., working under limited supervision, providing guidance to Community Paramedics (CP), EMS crews, and healthcare providers. The goal is to ensure patients receive appropriate care and resources, avoiding unnecessary hospital visits. The PC makes informed decisions, manages emergency situations, and transfers information to 911. Candidates will rotate through a Community Paramedic unit to maintain skills.
The Community Paramedic (CP) provides immediate or scheduled medical supervision and treatments outside the hospital setting, collaborating with healthcare services to care for patients in place, unless transport is necessary. The primary focus is on primary and urgent care, with the ability to deliver emergency treatment if needed.
As an ACP, responsibilities include assessing scenes and patients, transporting patients, administering medications, and leading the ALS crew. The environment is demanding, with exposure to various weather conditions, hazards, and physical exertion such as lifting up to 105 lbs, and working in awkward positions.
Required: Non-restricted registration as an Advanced Care Paramedic with ACOP, BCLS-HCP certification, valid Class 4 Alberta Operator License or equivalent, recent F.A.R.E. test completion, and ability to perform full operational duties.
Additional: Maintain clinical competencies, support patient transfers, demonstrate good work performance, and collaborate effectively. Must have completed an accredited ACP program and be registered with A.C.P.
Preferred: Community Paramedic experience, patient coordinator experience, community health or urgent care background, and leadership experience.
