Lifemark is seeking a part-time Care Coordinator responsible for enhancing patient experiences through administrative support and coordinated care. This role involves managing patient interactions, scheduling, and ensuring seamless service in a collaborative clinic environment.
Position Title: Care Coordinator
The Receptionist / Care Coordinator is responsible for fostering a strong relationship between the clinic and its customers, including patients, staff, funders, and referral sources. This is achieved by efficiently providing administrative support and coordinating clinic activities with friendly, enthusiastic, and professional service to all patients, team members, and external customers.
The Receptionist is responsible for all aspects of patient intake, data entry, answering the phone, outbound calling, receiving faxes and emails, coordinating referrals, as well as invoicing and payments.
