Central London Community Healthcare (CLCH) is proud to provide a wide range of community health services, and our South West London Division is seeking a motivated Occupational Therapist to join the Merton Adult Community Neuro Rehabilitation Team.

This is an exciting opportunity to work within a supportive multidisciplinary team delivering specialist, evidence-based rehabilitation for people living with stroke and a range of neurological conditions in the community, including long term neurological conditions.

You will play a key role in helping individuals maximise their independence, achieve meaningful rehabilitation goals and improve their quality of life. The role offers opportunities to contribute to service development, innovation and continuous improvement while further developing your own clinical expertise through ongoing professional development and shared learning.

Working collaboratively with therapy, nursing and wider health and social care colleagues, you will help deliver integrated, person-centred care that supports positive outcomes for patients and their families.

As a Specialist Neuro Occupational Therapist within the Merton Community Neuro Rehabilitation Team, you will:

  • Work as an active member of a multidisciplinary team specialising in stroke and neurological rehabilitation.
  • Deliver specialist Occupational Therapy assessment, treatment and rehabilitation for adults with complex neurological conditions.
  • Manage a caseload of patients with complex physical, cognitive and emotional needs.
  • Develop and implement person-centred rehabilitation plans that support independence and maximise recovery.
  • Contribute to timely, coordinated and safe discharge planning in partnership with patients, carers and relevant professionals.
  • Support the development of the Occupational Therapy service and wider team to promote integrated, evidence-based practice.
  • Provide specialist advice, guidance and education to health and social care professionals, patients, carers and families.
  • Work collaboratively with multidisciplinary colleagues to deliver high-quality community rehabilitation services.
  • Support and supervise junior staff, support workers and students, contributing to their learning and development.
  • Maintain professional standards through reflective practice, continuous professional development and participation in service improvement activities.

This role requires excellent clinical reasoning, effective communication skills and a commitment to delivering high-quality, compassionate care within a community setting.
