Talent Acquisition Specialist / Human Resources Associate
Lead, Quality, Safety and Risk

Our mission is to provide high-quality, community-based residential, transitional, and respite care for individuals and their families living with complex care needs at our facilities in Toronto, Etobicoke, North York, Woodbridge, and Aurora.
The Lead, Quality, Safety and Risk reports directly to the Manager, Quality, Safety, and Risk and works in partnership with teams to support staff and leadership across the locations to apply the knowledge of Ministry of Children, Community and Social Services (MCCSS) and Accreditation standards to ensure client records, physical environments, policies, and other items align with Ministry and Accreditation Canada requirements. The lead supports Safehaven’s Accreditation Continuous Readiness strategy and the organization’s adherence to QAM and Pediatric licensing requirements across the organization. In addition, this role is responsible for participating in the collection of data for specialized quality assurance audits and prepares quarterly reports for the Board of Directors and the Board Quality, Safety and Risk Committee.
ESSENTIAL FUNCTIONS / AREAS OF ACCOUNTABILITY
Ministry QAM and Licensing
- Support staff with education and application of client record reviews; providing constructive feedback that guides staff to complete the necessary changes (if applicable)
- Responsible for weekly onsite visits at the locations to provide support to the frontline staff
- Responsible for ensuring that all services and supports are provided in accordance with the Quality Assurance Measures (QAM), Services and Supports to Promote the Social Inclusion of Persons with a Developmental Disability Act (SIPPDA), and Pediatric Licensing mandated by the Children Youth and Family Services Act (CYFSA)
- In collaboration with the nurse educator team, provide training on General Service Plans (GSP), licensing standards, primary care duties and other materials related to ministry standards
- Responsible for ensuring that the Manager, Directors, and other members of the leadership team are kept current with any changes to QAM and CYFSA legislation in a timely manner
- Responsible for ensuring that staff are kept current with any changes to QAM or CYFSA legislation; ensure that education is provided to staff
- In partnership with the Lead Admin and Special Projects, ensure that all governance documents meet QAM / Pediatric standards
- In collaboration with the Client Care Coordinators (CCC) and Human Resources (HR), the lead updates and ensure standards are current and disseminated to appropriate individuals; reviews annual policy sign-off documents and ensures they meet QAM, Pediatric, and OHS standards
- In partnership with other members of the leadership team, prepare regular reports for various audiences (Board of Directors, Leadership team, locations, etc.)
- Support the review, feedback and development of policies related to QAM, Pediatric licensing, OHS to achieve 100% compliance, as per SIPPDA and CYFSA legislation
- In collaboration with other members of the leadership team, perform bi-monthly onsite mock audits and inspections
- In collaboration with the CCCs, ensure licensing binders are kept up to date and in compliance with licensing standards
- Play a key role in the implementation of Ontario’s Quality Standards Framework (QSF) to the organization’s policies, procedures, education / training, and applicable documentation
- Primary liaison with MCCSS Program Advisors and Licensing Specialists during prescheduled and unannounced inspections and audits of client documentation, staff and delegate interviews, review of HR files, policies and procedures, physical inspection and property-related documentation, and review of Board records. Address any non-compliances within the required timeframe
- Support MCCSS Serious Occurrence Reporting – Residential Licensing (SOR-RL)
Client Safety
Support administration of incident reporting and the client safety incident review and analysis processCollaborate with clinical teams in proactively identifying potential client safety priorities, and support problem-solving, in consultation with appropriate team members / stakeholdersSupport the development of curriculum and deliver ongoing training and orientation to staff, highlighting the importance of incident reporting in sustaining a safety cultureSupport Safehaven’s risk management program and frameworkSupport the identification and mitigation of risks with a focus on improving client / family experience and safety, and the overall quality of care and serviceSupport the planning, implementation and monitoring of the Accreditation Continuous Readiness strategy to embed compliance with Accreditation Standards, Required Organizational Practices, and surveys into the organization’s day-to-day activities and cultureAct as a resource to Safehaven staff providing advice and support to all clinical and non-clinical programs and departments and to help them understand and meet Accreditation Canada Standards and ROPsDevelop educational materials to support the advancement of quality / learningOccupational Health and Safety (OHS)
Accountable for ensuring leadership is kept current of changes to the Occupational Health and Safety Act and prepares documents to inform staff across the organizationQualifications
Bachelor’s degree in a healthcare-related field preferred2-3 years of relevant experience in quality, safety, and regulatory compliance, in healthcare preferred (including knowledge of MCCSS regulatory requirements and / or Accreditation Canada standards)Experience as a Personal Support Worker, Residential Support Worker, Developmental Services Worker, Bachelor of Social Work, Registered Practical Nurse, or Registered Nurse preferredExperience working with individuals with development disabilities preferredOther
