Quality Improvement Lead (1.0 FTE, Regular)

TVFHT is one of Ontario’s largest and most dynamic family health teams, serving patients across over 21 sites in four counties. Our interdisciplinary teams including physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians, and other allied professionals work together to provide comprehensive, patient-centered care. The Quality Improvement (QI) Lead plays a central role in advancing the organization’s commitment to excellence in healthcare delivery through data-driven quality improvement initiatives. This role is responsible for leading and overseeing all departmental activities related to QI, including data benchmarking, analytics, performance reporting, and program operations. The QI Lead ensures the integrity and consistency of data, supports the development and execution of improvement strategies, and actively engages with staff at all levels to promote a culture of quality and continuous improvement.

Qualifications:

• Bachelor’s degree in a healthcare, business, or data-related field required.
• Master’s degree in health administration, public health, statistics, data science, or a related discipline preferred.
• Minimum of 5 years of progressive experience in healthcare quality improvement, performance measurement, or a related field.

Key Responsibilities:

• Oversee the work and activities of all departmental team members, ensuring alignment with organizational priorities and QI best practices.
• Lead and manage benchmarking activities, interpret performance metrics, and provide analytic insight into healthcare trends and data variation.
• Oversee the selection, use, and optimization of tools and mechanisms used for data collection, extraction, and reporting, ensuring consistency and accuracy.
• Lead the development of QI reporting including the Quality Improvement Plan (QIP), quarterly performance management reports, and contributions to the Annual Operating Plan.
• Support and oversee operational elements of group programming and initiatives related to clinical and organizational improvement.
• Foster engagement with staff through various communication and connection opportunities (e.g., town halls, newsletters, small group sessions) to advance quality-focused goals.
• Report to and liaise with the Quality Committee of the Board on progress, challenges, and performance against QI objectives.
• Ensure consistency and quality of extracted data; demonstrate a robust understanding of sources of data variation and apply this insight to improve performance interpretation and strategic planning.
• Strong knowledge of healthcare quality frameworks, measurement, and regulatory requirements.
• Excellent communication and presentation skills, with the ability to engage diverse stakeholders.
• Highly detail-oriented with strong organizational and analytical skills.
• Proven time management capabilities and the ability to meet tight deadlines while managing multiple priorities.
• Advanced proficiency in data analysis tools and platforms (e.g., Excel, Power BI, statistical software).
• Ability to synthesize complex data and communicate actionable insights.
• Experience working collaboratively in cross-functional teams and with leadership.
• Advanced skills with the use of and extraction of data from Electronic Health Record software
• May require flexible scheduling with occasional evening work
• This position offers a hybrid work arrangement, allowing for a blend of remote work and in-office/on site collaboration
