Job description

Reporting to the Manager, Quality and Practice, the Quality Improvement and Patient Safety Consultant has demonstrated experience in leading processes related to quality improvement, patient safety, accreditation, and critical incident management. This role supports organizational initiatives by collaborating with leadership and staff to develop policies that ensure high-quality care, minimize patient harm, and foster continuous improvement.

Key responsibilities include implementing accreditation standards, supporting the Quality Improvement Plan (QIP), analyzing incident data and care trends, and facilitating evidence-based improvements. The consultant collaborates with stakeholders to implement solutions, map new processes, and evaluate the impact of safety initiatives, including accreditation readiness activities such as mock surveys and evidence submissions.

This role also involves supporting critical incident management by coordinating root cause analyses, leading debriefings, and translating learnings into system improvements. A focus on embedding a Just Culture approach is essential for proactive risk mitigation.

The position is responsible for advancing quality, accreditation, and safety agendas through evidence-based strategies, leading accreditation activities, and supporting the QIP to improve patient outcomes. The consultant works closely with clinical leaders, staff, and interdisciplinary teams to ensure alignment with strategic priorities and standards.

Accountabilities include:

  1. Analyzing safety incidents, quality indicators, and accreditation metrics to identify trends.
  2. Leading critical incident reviews, root cause analyses, and accreditation tracers to drive improvements.
  3. Developing policies and strategies with clinical leaders to enhance quality and safety.
  4. Monitoring the QIP to achieve measurable patient care improvements.
  5. Engaging staff in safety initiatives through education and participation.
  6. Providing project management and data analysis support for quality and risk initiatives.
  7. Maintaining systems for incident reporting, evidence collection, and QIP tracking.
  8. Participating in quality, safety, and accreditation committees.

Qualifications:

  • Undergraduate degree in a related field (e.g., health care administration, health professions).
  • College degree or certification in clinical practice, risk management, or healthcare administration.
  • Minimum three years of experience in clinical risk management, quality improvement, accreditation, or related healthcare fields.
  • Experience with critical incident management or patient safety within the last three years is preferred.
  • Member of a regulated college in Ontario is preferred.
  • Certified Risk Management designation is an asset.
  • Understanding of acute care environments and quality improvement methodologies.
  • Strong computer, data management, and statistical skills.
  • Ability to manage multiple stakeholders and priorities effectively.
