2. A resume outlining qualifications and experience
3. A copy of the applicant’s Ontario College of Teachers’ Certification in good standing
4. The name of three (3) professional references, one of which must be a current or most recent immediate supervisor, along with telephone numbers, email addresses, and a written authorization from the applicant permitting the Board to contact these references.
