Psychosocial Spiritual Care Clinician - Contract Position

Position Summary

Ontario Health At Home has contracted Huron Hospice to provide psychosocial spiritual support to people in Huron Perth who are palliative. Huron Hospice is looking for one or two skilled and compassionate individuals to provide this care. Since the geography is large, we may contract two clinicians half-time, one in Perth and one in Huron. Alternately, we may contract one person fulltime to cover the full area. Normally, referrals come from home care nurses or others who are asked by their patients for this care. This is a pilot period which will be evaluated and may or may not be extended.

The Psychosocial Spiritual Care Clinician is concerned with the psychological, emotional and spiritual well-being of persons who are dying and their families, as they deal with issues of self-esteem, adaptation to the illness and its consequences, social function and relationships, communication, and spiritual suffering. Assisting and supporting conversations essential for end-of-life decision making is included in this role. The clinician will support the practices of others which respects the diversity of religious practices and beliefs of those being served, including those who have no religious tradition or spiritual practices.

Key Responsibilities

  • Client Service Delivery: Receives, prioritizes and takes timely action regarding new referrals.
  • Client Service Delivery: Develops and builds relationships with patients and their circle of care, by utilizing various forms of communication including face to face, video and telephone visits.
  • Client Service Delivery: Assists with referrals to additional community supports such as housing and bereavement services.
  • Patient Safety: Maintains a daily focus on patient safety as everyone’s responsibility.
  • Patient Safety: Complies with patient safety initiatives, e.g. hand hygiene protocols.
  • Patient Safety: Keeps alert to patient hazards and takes prompt action to eliminate unsafe conditions, including not exposing patients or co-workers to your illness.
  • Patient Safety: Complies with the Ontario Health At Home, South West Code of Conduct, Occupational Health and Safety Act, applicable regulations and procedures, e.g. wearing appropriate personal protective equipment.

Qualifications And Experience

  • Degree in one of the following: Social Work, Ministry and Spirituality, Theology or equivalent. Masters preferred.
  • Certified Spiritual Care Practitioner in good standing (Canadian Association for Spiritual Care-CASC) and/or member in good standing of the College of Psychotherapists of Ontario or demonstrated equivalent combination of life experience and training.
  • Certificate in the Fundamentals of Hospice Palliative Care or the commitment to obtain.
  • Additional training obtained through CAPE and/or SCOPE preferred.
  • Minimum of five (5) years related spiritual care experience.
  • Effective planning, organizing and evaluation skills to manage multiple patients, provide information reports and take corrective action as required.
  • Extensive experience supporting individuals and families facing a life-threatening illness, as well as providing bereavement support.
  • Proven knowledge of palliative care is a requirement.
  • Practical knowledge of relevant legislation (e.g. privacy legislation, etc.)
  • Knowledge of community resources (e.g., services and programs) and roles of health care professionals.
  • Knowledge and application of professional college standards in relation to Clinical practice, if applicable.
  • Ability to relate theology and behavioral science to the practice of spiritual care.
  • Knowledge of various models of spiritual care and their appropriate use in a variety of practice contexts.
  • Expert knowledge and experience providing practical assistance and information that pertains to future planning including: advance care planning and end of life rituals, Powers of Attorney, Wills, Compassionate Care Benefits, and insurance policies.
  • Ability to use MS Office applications (e.g., Word, Excel, PowerPoint, etc.).
  • Demonstrated proficiency with computer navigation and data inputting.
  • Experience with file sharing software.
  • Retains a license to drive in the Province of Ontario and has consistent access to a vehicle in safe working order that is licensed appropriately.
  • Proficiency in English required.
  • Proficiency in French is an asset.

NATURE AND SCOPE

Effective listening and strong communication and interpersonal skills are necessary to work with diverse patient groups and cultures, their families and health care providers in order to ensure best possible care is provided to the patient.

Ability to handle challenging situations in a respectful and ethical manner, seeking manager and peer input as needed.
