Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and Fraser Health's Mission and Values, the Psychiatric Liaison Nurse works collaboratively with the interprofessional care team to provide person-centered, recovery-oriented care. Responsibilities include performing intake assessments, offering crisis intervention, and providing input into the care of psychiatric and concurrent disordered patients in the Emergency Department. The role also involves ensuring effective patient flow, adapting care plans, and liaising with community mental health services.

Responsibilities

  1. Screen psychiatric referrals, gather background information, and determine service priorities. Collaborate with Emergency Department staff to identify potential psychiatric referrals.
  2. Perform psychosocial assessments, collect collateral information, and complete emergency psychiatry patient assessments for triaged patients.
  3. Develop care plans with final dispositions authorized by physicians or psychiatrists; provide crisis counseling to patients and families, including short-term follow-up.
  4. Provide short-term follow-up treatment and case management at the Mental Health Centre as needed.
  5. Communicate with team members regarding patient care, organize interprofessional conferences, and involve patients and families in care planning.
  6. Advocate for patients by connecting them with community resources and services.
  7. Document assessments, care plans, interventions, and outcomes according to standards and procedures.
  8. Maintain electronic contact sheets and participate in patient care conferences for discharged psychiatric patients.
  9. Facilitate transition to aftercare and act as patient advocate by assisting access to community resources.
  10. Guide hospital staff on psychiatric care practices, violence de-escalation, and safety policies.
  11. Participate in quality improvement activities, including data collection and practice adjustments.
  12. Provide input on policies and procedures, and suggest operational improvements.
  13. Identify personal learning needs and pursue ongoing education to enhance competencies.
  14. Maintain a safe environment by adhering to safety and emergency protocols.
  15. Contribute to staff training and mentorship as appropriate.
  16. Perform other related duties as assigned.

Qualifications

Education and Experience

Graduation from an approved Nursing or Psychiatric Nursing program, current registration with BCCNM, and at least one year of recent experience in mental health and substance use settings or an equivalent combination of education and experience.

Skills and Abilities

  • Assessment and nursing care skills, including crisis intervention.
  • Knowledge of mental health and substance use treatments, psychopharmacology, and relevant legislation.
  • Strong communication, counseling, and group facilitation skills.
  • Ability to promote person-centered, culturally safe care, and advocate for patients.
  • Organizational, collaboration, and evidence-based decision-making skills.
  • Proficiency with related equipment and computer applications.
  • Physical ability to perform position duties.
