A leading home health care service provider in Cornwall, Ontario, is looking for a dedicated PSW/HCA to provide compassionate care to patients. This full-time role requires a PSW program completion and a reliable vehicle for community home care. Ideal candidates will have experience in long-term care and exceptional communication skills. Qualifications

  • Completed a PSW Program or HCA/CCA certification meeting the standards required for the position.
  • Experience in Long Term Care or community home care settings preferred.
  • Knowledge of care planning systems and documentation.
  • Ability to communicate positively with staff, residents/patients, and families.
  • Ability to provide care in accordance with legislation and to read, write, and understand English.

  • Provide direct personal support and care to patients.
  • Assist with ambulation, feeding, bathing, and grooming.
  • Fulfill physical, intellectual, emotional, social, and spiritual needs of patients.

Job Description

The PSW/HCA/CCA provides direct personal support and care to patients to fulfill their physical, intellectual, emotional, social, and spiritual needs in accordance with policies, procedures, and work routines. Responsibilities include providing direct care such as ambulation, feeding, bathing, and grooming.

For Union Jobs, please refer to the collective agreement and include the following information in the Job Description:

  • Union Name

  • Union Classification (Title): PSW / HCA as applicable
  • Salary: As per Collective Agreement

Access to a reliable vehicle in community home care is required.

For both Non-Union and Union jobs, the following qualifications apply:

  • Completed a PSW Program or HCA/CCA certification meeting the standards required for the position
  • Experience in Long Term Care or community home care settings, with exposure to dementia and palliative care preferred
  • Knowledge of care planning systems and documentation
  • Ability to communicate positively with staff, residents/patients, and families
  • Ability to provide care in accordance with legislation, job routines, nursing care plans, and to read, write, and understand English verbal and written instructions
  • Experience working independently in Community Home Care is an asset
