Reporting to the Project Director, Homelessness Initiatives and Prevention Services, the Project Manager will collaborate with healthcare partners to create referral pathways between primary, mental health, emergent, and community healthcare and shelter providers, aiming to improve health and housing outcomes for homeless individuals citywide. The role involves providing leadership and guidance to resolve care access barriers across the healthcare system.

Major Responsibilities
  • Develop and maintain strategic relationships with healthcare providers, government officials, and internal management to address systemic gaps in healthcare access.
  • Lead and support the development of cross-sector agreements, policies, and protocols to improve healthcare access for homeless individuals.
  • Negotiate and implement innovative solutions involving multiple stakeholders to increase healthcare access.
  • Represent the division in meetings with diverse healthcare stakeholders, including city divisions, community groups, agencies, and officials.
  • Advocate for improved health outcomes across diverse communities experiencing homelessness.
  • Support evaluation and implementation of the Homelessness Health Services Framework by identifying barriers to care.
  • Develop and execute plans supporting new programs and system changes.
  • Prepare reports and briefing notes for senior leadership.
  • Analyze emerging issues and recommend innovative organizational or programmatic solutions.
  • Manage data collection strategies to support healthcare access solutions.
  • Facilitate knowledge transfer to empower site-level leadership.
  • Lead project planning and development groups, formulating and recommending new program features and implementation plans.
  • Supervise and motivate project team members, fostering teamwork, high-quality work, and innovation.
Ideal Candidate
  • Post-secondary education in health, social sciences, public administration, or equivalent experience.
  • Experience building strategic partnerships and assessing programs for diverse communities.
  • Proven project management experience in health and social services sectors, including policy development, evaluation, and community development.
  • Experience in advisory roles involving government and community stakeholders.
  • Leadership experience managing professional teams.
  • Ability to align and gain support from senior leadership for new initiatives.
  • Strong report writing and communication skills.
  • Political acumen, negotiation, and conflict resolution skills.
  • Experience leading change management initiatives.
  • Strategic and analytical thinking skills, with capacity to handle complex issues.
  • Knowledge of health inequities, social determinants of health, and homelessness issues.
  • Familiarity with relevant legislation, policies, and city guidelines.
