Hours: 37.5 per week
Status: Permanent Full-Time (contingent on funding)
Connected Care is a strategic business unit at UHN focused on developing and delivering innovative models of care. Our work focuses on transformative care experiences for patients, essential care partners and care provider teams. We break down barriers to change, bringing together policy makers, funders, thought leaders, clinicians and patients to the table to build solutions together.
Connected Care partners with a number of groups to spread and scale transformative change models, including partnering with OpenLab to lead the NORC Innovation Centre (NIC). The NIC aims to transform the way health care is delivered and experienced by seniors living in the community. The Centre’s mission is to advance a 21st century model of integrated care in NORC buildings across Toronto, and to develop health, social, and technology interventions that support healthy aging. The initial focus for the Project Director will be to lead the spread and scale of UHN’s NORC model of care across Ontario.
Position Summary:
Reporting to the Sr. Director, UHN Connected Care, the Project Director will be responsible for leading the implementation and operations of system transformation initiatives. The successful candidate will be responsible for working with Ontario Health Teams, delivery and care partners across the health system to deliver new care models to support Ontarians.
