Yee Hong is one of the foremost providers of culturally appropriate senior care in the country, delivering high-quality resident experience and care to Chinese and other Asian seniors since 1994. The organization operates 805 long-term care beds and 308 seniors’ apartment units, with an additional 224 long-term care beds opening in a new home by June 2026. In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.
Core Function:
Under the overall direction of the Program Manager/Program Coordinator, participates in the planning and implementation of appropriate social activities, providing health monitoring and emergency response services to the residents of the building.
Qualifications:
1. Diploma in Social Services, Social Work, health care, activation, or equivalent
2. Minimum 2 years experience in social services or health care, working with seniors in a community setting
3. Current, valid C.P.R
4. Satisfactory Police Reference Check (with vulnerable sector screen) result
Language Requirement:
Good oral and written communication skills in English and Chinese
Responsibilities:
1. Plans, organizes, implements and evaluates activity programs; designs, develops and implements the monthly program schedule
2. Coordinates meal and transportation services for clients/residents
3. Monitors overall health status and keeps up-to-date health records of clients/residents
4. Complies with risk management policy and infection control to ensure health and safety for clients and staff
5. Assists the senior/program coordinator in the development and update of policies, procedures for the program
6. Coordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteer
7. Maintains up to date and accurate files, records and statistics on the program
8. Ensures efficient and responsible use of resources and maintains accurate record of fees and expenditure
9. Provides emergency response to emergency calls from clients/residents of the building
10. Assists the senior/program coordinator in preparing home support workers schedules in accordance with established shift scheduling guidelines
11. Effectively promotes the home support services to residents of the building
12. Works with the whole team to create and promote a safe and caring living environment for the residents
13. Other duties as assigned
