Job summary

South Riverdale Community Health Centre in Toronto is seeking a Program Manager, Chronic Disease to coordinate DECNET, Tele-Ophthalmology, Community Dietitians and Choose Health programs. You will supervise a diverse team of staff, volunteers and students, including hiring, performance management and disciplinary decisions in consultation with the Director.

Working onsite in east Toronto, you will lead program planning, quality improvement, evaluation and reporting, ensuring client-centered

Qualifications

  • Graduate degree or significant experience in health care, social services, or related field.
  • 2-5 years experience coordinating programs and supervising staff.
  • Registered Regulated Health Care Professional preferred.
  • Current college membership under the Regulated Health Professions Act, 1991 where applicable.
  • Ability to supervise and support staff to ensure client-centred, effective and efficient work processes.

South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Program Manager, Chronic Disease is responsible for the coordination and oversight of DECNET, Tele-Ophthalmology, Community Dietitians, and Choose Health programs. In this role, the Program Manager is further responsible for the supervision and support of a diverse team of staff, volunteers and students, including hiring, performance management, disciplinary action and decisions regarding termination of employment. Leading these programs, this position plays a lead role in responding to changing needs and trends through the planning, development, implementation and evaluation of programming to meet the needs of communities in east Toronto

Working onsite is required.

Responsibilities:
Talent Management and Supervision of Staff
  • Direct and manage the work of interdisciplinary teams (including peer workers, students and volunteers)
  • With support from the Director, conduct all required recruitment activities to ensure the programs are appropriately staffed
  • Provide supervision and ongoing guidance, feedback, recognition and coaching of the program staff
  • Support staff development opportunities and engage staff appropriately in decision making
  • Conduct probationary and annual performance reviews
  • Support the development and maintenance of a strong team
  • Support staff wellness activities
  • Work with program staff to resolve problems as they arise
  • Monitor attendance and address concerns
  • Ensure that staff’s statistical encounters, charting, monthly time records and all administrative tasks are completed in a timely manner
  • Address performance issues as they occur and make effective recommendations with respect to disciplinary steps up to and including termination, in consultation with the Director
  • Ensure all appropriate employee information is tracked and documented according to agency policies
Program Management
  • Ensures program services meet national/best practice guidelines for diabetes care, coordinates chart audits / surveys / evaluations, etc.
  • Coordinate daily/weekly/monthly/annual priorities, work plans and workflow for all identified programs, in conjunction with the Director
  • Ensure supportive, strengths-based, trauma-informed, anti-racist/anti-oppressive social justice and harm reduction approaches are utilized in all aspects of program planning and direct service
  • Ensure the successful coordination of client-centered services
  • Develop targets, timelines and processes to successfully manage tasks and workflow
  • Address day to day program and client related issues and ensure these are resolved in a clear, professional, timely manner
  • Develop and coordinate inter-professional team approaches to meet the complex short and longer-term needs of clients/participants and to address the social determinants of health including utilizing crisis intervention, counselling, case management and referral as needed.
  • Monitor, support, train and evaluate the ongoing development and implementation of the Diabetes, Tele-Ophthalmology, Community Dietitians and Choose Health related services
  • Identify and respond to increased risks, trends and gaps in programs, by offering services and support that respond to community feedback and meet diverse needs
  • Guide and support program staff in responding to and providing services
  • Develop and maintain collaborative team programming and partnerships within Community Health and Chronic Disease and other inter-disciplinary teams and programs at SRCHC, as well as with community partners and other service agencies
  • Develop program workplans in consultation with team, while aligning with organizational strategic plan
  • Lead quality improvement initiatives
  • Support accreditation and standards recognition activities
  • Maintain records, reports and other statistical information, and provide accurate and timely reports, stats, stories of impact to ensure program accountability
  • Make recommendations related to supplies and other purchases and monitors budgets and expenditures in conjunction with the Director
  • Ensure all funding reports are submitted as per accountability agreements and prepares funding proposals for review by Director
  • Develop, and coordinate effective print and electronic promotional information and materials, and strategies to outreach to individuals who may benefit from the Diabetes, Tele-Ophthalmology and Choose Health programs; organize and give presentations for diverse audiences
  • Develop outreach strategies with team to meet client and community needs

Graduate degree or significant experience in health care, social services, adult education or related field

  • 2-5 years progressive experience coordinating programs, managing and supervising staff
  • Registered Regulated Health Care Professional preferred
  • Current college membership as regulated under the Regulated Health Professions Act, 1991 maintained as applicable
  • Demonstrated ability to supervise and support staff to ensure client-centred, effective and efficient work processes
  • Familiarity with, and experience in, self-management and chronic disease programming
  • Experience working with vulnerable and diverse communities
  • Demonstrated ability in project coordination and management and program development
  • Experience with outreach initiatives and building strong partnerships
  • Demonstrated experience in integrating initiatives across multiple sectors; with a range of different organizations and individuals
  • Solid interpersonal, supervisory and critical thinking skills
  • Strong organizational skills
  • Excellent verbal and written communication skills
  • Proficiency in the use of computer technology and various software applications
  • Ability to work independently and as a member of a multi-disciplinary team
  • Police clearance documentation will be required for employment
  • Proof of double COVID-19 vaccination status will be required for employment
