Job Description

This position is responsible for developing and implementing individual treatment/support plans from admission through discharge and follow-up. The role includes coordinating care, support, and appointments/services, maintaining regular contact with families and case managers/service coordinators, and providing ongoing progress reports, service documentation, data collection, analysis, and healthcare/medication documentation. The individual in this role acts as an advocate for supported individuals and communicates routinely with families, healthcare professionals, and other supports. They provide direction, oversight, training, and feedback to staff supporting individuals according to the treatment/support plans.

Qualifications

  • At least two years of supervisory or management experience in developmental disabilities services or mental health setting.
  • Bachelor’s degree in a Human Services field (e.g., healthcare, social services, education, behavioral science, counseling).
  • Five years of experience in developmental disabilities or mental health, or an equivalent combination of education and experience.
  • Good driving record; proof of current driver's license and insurance required.
  • Ability to serve as an appropriate role model, demonstrating mature behavior and sound decision-making.
  • Working knowledge of federal and state regulations for facilities and programs serving individuals with developmental disabilities.
  • Ability to speak to groups of staff, articulate clearly, and demonstrate enthusiasm toward staff and individuals supported.
  • Flexibility to work evenings, weekends, and holidays as needed to meet program needs.
