Job summary

Acclaim Health is seeking a compassionate and organized Program Coordinator to support our respite services within our Adult Day Program at Patty’s Place on Speers Road. This full-time role runs Monday to Friday, 11:30 am to 8:00 pm with an on-call rotation and on-site supervision.

The position coordinates daily programming, assists participants with diverse needs, and ensures a safe, engaging environment while collaborating with families and staff to meet individual goals.

Qualifications

  • Diploma in Gerontology, Social Services or equivalent.
  • CPR-Basic Rescuer Level certification.
  • Ontario driver’s license with access to a vehicle and appropriate insurance.

Responsibilities

  • Admissions and check-in/check-out for overnight respite services.
  • Monitor client status and update person-centered service plans.
  • Provide leadership to staff during emergencies and on-call rotations.
  • Coordinate with Recreation staff for therapeutic programming.
  • Assist clients with activities of daily living and meal planning.
  • Maintain confidential electronic client files and risk management.

This role is responsible for coordinating daily programing, supporting participants with diverse needs, and ensuring a safe and engaging environment.

The ideal candidate is a strong communicator who can balance administrative responsibilities with hands‑on support, collaborate with staff and families, and adapt programming to meet individual goals.

Acclaim Health announces the availability of a Full-Time Program Coordinator – Respite position to work at Patty’s Place (located at 2250 Speers Road). This position will work Monday-Friday 11:30-8:00 pm and will participate in a rotation for supervisory on call, which will require shift flexibility to work on‑site on a Saturday on their scheduled week.

Pay Rate: $26.91 - $28.56 per hour

PRIMARY FUNCTIONS:

  • Works collaboratively with Supervisor, Respite to conduct timely client admissions into the overnight respite service and client check-in/check-out procedures.
  • Assesses and monitors client status ensuring that collaborative client-centered service plans are in place and current.
  • Provides leadership to staff when clients are exhibiting responsive behaviours and/or are experiencing emergencies.
  • Participate in a rotation for supervisory on call support (evenings and weekends).
  • Ensures overnight respite services are aligned with best practices such as age-friendly, dementia care and for adults with special needs.
  • Ensures communication linkages between the overnight respite services and the adult day program services.
  • Coordinates with Recreation Staff the therapeutic recreation planning suited to client interests. Actively participates in the delivery of the programming along with Recreation staff, students, and volunteers.
  • Assists clients with activities of daily living including meal preparation, eating, toileting and mobility as required.
  • Manages food ordering and meal planning for overnight respite clients.
  • Manages the supervision, training, mentoring and development of direct reports.
  • Develops and monitors daily staff plan, ensuring adequate coverage.
  • Assists with and participates in special events for clients/family caregivers.
  • Maintains accurate and confidential electronic client files.
  • Identifies, minimizes, and manages client risk in accordance with risk management and incident reporting standards, policies, and procedures.
  • Other duties as required.

QUALIFICATIONS & EDUCATION:

  • Diploma Community College, Gerontology, Social Services or equivalent
  • Certification in CPR-Basic Rescuer Level
  • Valid Ontario driver’s license, use of vehicle and insurance per the Acclaim Health policy

EXPERIENCE & SKILLS:

  • Minimum 2 to 3 years demonstrated experience working with adults with special needs, frail seniors and cognitively impaired persons and their families.
  • Effective verbal, written communication and interpersonal skills.
  • Demonstrated ability to coach and lead staff.
  • Knowledge and understanding of community resources and the Halton healthcare system.
  • Strong assessment skills.
  • Strong customer service skills.
  • Effective decision-making, problem-solving and organizational skills.
  • Demonstrated knowledge in quality/risk management.
  • Proven ability to prioritize and meet deadlines.
  • Proficient in Microsoft Office and AlayaCare (an asset).
  • Knowledge and skilled in development of teams and staff development.
  • Demonstrated ability to take initiative, work independently and collaboratively as a team member.

Individual must be able to multi-task and set priorities in order to meet the needs of the program. This position requires computer work. There can be times when concentration can be difficult due to frequent interruptions. There are periods of stress when having to meet very tight deadlines. Client/caregiver interaction is required. Required to assist with client ADL skills and transfers. May be required to wear protective clothing in certain circumstances. Travel is required on occasion.
