Primary & Community Care Social Worker - Burnaby

Salary : CAD $42.27 - $52.81 per hour

Job Summary

We are seeking a dedicated full-time Social Worker to join our Primary Care team in Burnaby. Responsibilities

  • Conduct comprehensive social work and psychosocial assessments of clients and their families.
  • Provide therapeutic services and coordinate care plans with clients, families, and the interdisciplinary team.
  • Support seamless care across healthcare settings, including hospital and community services.
  • Partner with community agencies and health providers to manage at-risk individuals and facilitate referrals.
  • Maintain patient records and documentation in accordance with standards and regulations.
  • Develop and deliver educational sessions, support groups, and training.
  • Participate in local committees and research activities to improve patient care services.
  • Perform other related duties as assigned.

Qualifications

  • Current registration with the BC College of Social Workers.
  • At least 2 years of recent experience in a complex healthcare environment.
  • Valid BC Driver's License and access to personal vehicle (may be required).

Skills and Abilities

  • Knowledge of counseling, assessment, and health legislation.
  • Excellent communication and organizational skills.
  • Ability to work independently and within a team.
  • Understanding of First Nations, Metis, and Inuit health and culture.
