Job summary

Northern Health is seeking a Social Worker to join the Primary Care department in a dynamic team. This role focuses on psychosocial assessment, care planning, and intervention for individuals and families within Primary Care Homes and interprofessional teams.

Responsibilities include direct psychosocial care, counselling, navigation of community resources, and advocacy to support client self-management. A BC driver’s license and active registration are required.

Qualifications

• Bachelor's degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting or equivalent.
• Current registration with the British Columbia College of Social Workers.
• Current valid BC driver's license.

Responsibilities

• Provide direct psychosocial care, counselling and group work to individuals and families.
• Engage in system navigation and advocacy, education, and linkage to relevant resources.
• Collaborate with the interprofessional team to support client self-management.
• Maintain and document case notes, care plans, and progress updates.
