This is a rewarding opportunity to provide comprehensive, patient-centered care in a well-established outpatient setting.

Responsibilities

Position Overview:• Full-time position in an outpatient primary care clinic

• Polish-speaking candidates preferred• See patients for preventive care, chronic disease management, and acute visits• Collaborate closely with a team of physicians, APPs, and clinical support staff• Manage your own patient panel with opportunities for continuity of care• 40 hour/week schedule with minimal call responsibilityWhat We Offer:• Competitive compensation and comprehensive benefits package• Supportive, team-based environment with leadership committed to APP development• CME support and professional growth opportunities• Mission-driven organization with a strong focus on community health

Qualifications

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
