Job summary

Northern Health is seeking an experienced Occupational Therapist to contribute to the Primary Care Home model. The OT will work with interprofessional teams to assess, plan, and deliver care that aligns with patient goals in various settings, guided by professional standards and evidence-based practices.

This role emphasizes person- and family-centered care, collaboration, and use of Meditech and related tools to support outcomes and service delivery.

Qualifications

• Graduate of a recognized Occupational Therapy program.
• Registered with the College of Health and Care Professionals of British Columbia.

Responsibilities

• Provide initial and ongoing assessments and plan treatments.
• Collaborate with interprofessional teams to deliver care.
• Deliver care across physician practices, homes, facilities and communities.
• Apply evidence-informed practice to achieve patient goals.
